DPR-70 DOCKET NO. 50-272 UNIT NO. 1 SPECIAL REPORT 89-1 This Special Report addresses the inoperability of the main'steam line radiation monitoring cbanriels whicn have not been restored to operable status within seven (7) days. This report satisfies the reporting requirements of Technical Specification 3.3.3.1~ Action Statement 23 pursuant to Technical Specification 6.9.2. It is being submitted within fourteen (14) days as per the Action Statement.

Radiation Monitoring System Channels 1R46 A-E Inoperable For > 7 Days Due To Parts Availability Problem Event Date(s): 1/19/89 Report Date: 1/26/89 This report was initiated by Incident Report No.89-037.

CONDITIONS PRIOR TO OCCURRENCE:

Mode 1 Reactor Power 100% Unit Load 1150 MWe DESCRIPTION OF OCCURRENCE:

This Special Report has been prepared in accordance with Technical Specification 3.3.3.lB Action Statement 23 requirements. The Main Steam Line Radiation Monitors, 1R46 A-E, have been inoperable for grea.ter than seven days ~ * *

  • Technical Specification 3~3.3.lB Action Statement 23 states:

"With the nl.imber of OPERABLE Channels. less than required by the Minimum Channels OPERABLE requirements, initiate the preplanned alternate method of monitoring the appropriate parameter(s),

within 72 hours8.333333e-4 days <br />0.02 hours <br />1.190476e-4 weeks <br />2.7396e-5 months <br />, and:

1) either restore the inoperable Channel{s) to OPERABLE status within 7 days of the event, or
2) prepare and submit a Special Report to the Commission pursuant to Specification 6.9.2 within 14 days following the event outlining the action taken, the cause of the inoperability and the plans and schedule for restoring the system to OPERABLE status."

On January 12, 1989 at 0258 hour0.00299 days <br />0.0717 hours <br />4.265873e-4 weeks <br />9.8169e-5 months <br />s~ Radiation Monitoring System (RMS) channels 1R46 A-E were declared *inoperable to support planned maintenance of Chilled Water System cooling valves. Upon completion of the planned maintenance, the same day, the system was to be returned to operable status. However, Chilled Water System Flow indication was not received. Therefore, the system was not returned to operable status.

  • APPARENT CAUSE OF OCCURRENCE:

The root cause of the 1R46 A-E channels inoperability for greater than 7 days has been attributed to parts availability problems.

Investigation of the failure to indicate Chilled Water System flow

for the RMS channels has been attributed to failure of the flow switch. The switch is a Safety Related component .. No safety related spare switches were available on site. Therefore, a special purchase order had to be issued.

The flow switch was not in the PSE&G folio system. Therefore, the component was not kept in stock.

ANALYSIS OF OCCURRENCE:

The 1R46 A-E RMS channels are used to monitor main steam line radioactivity. This is done to identify significant primary to secondary leakage. The 1R19 Steam Generator Slowdown RMS channels and the 1R15 RMS air ejector channel corroborate the 1R46 channels' indications.

The 1R46 A-E channels provide alarm indication. They do not provide

  • any interlocking functions. The channels are cooled via the Chilled Water System. The Chilled ~ater System flow switch provides flow indication as well as 1R46 A-E main steam flow isolation.

CORRECTIVE ACTION:

The channels were returned to operable status on January 25 at 0130 hours0.0015 days <br />0.0361 hours <br />2.149471e-4 weeks <br />4.9465e-5 months <br /> upon.receipt and installation of a new Chilled Water flow indicator. The Technical Specification Action Statement was exited at this time.

The flow switch.will be added to the folio system with appropriate re-order points.
