Subject: Meeting Report No. 50-272/88-18; 50-311/88-18
An NRC requested Management Meeting was held in the NRC Region I Office, King of
Prussia, Pennsylvania on October 20, 1988.
The meeting was attended by
representatives of the Occupational Safety and Health Administration.
The
purpose of this meeting was to discuss recent NRC identified weaknesses in the
area of.Industrial Safety, Radiological Controls and the fall of a worker into
the Unit 2 Reactor Cavity on October 18, 1988. Also discussed was the adequacy*
of QA/QC oversight of outage activities.
The Meeting Report enclosed with this letter provides a summary of the topics
discussed~ The meeting was beneficial in allowing us to be'tter understand your
Industrial Saf~ty Program and planned enhancements to this program, the apparent
circumstances leading to the fall of the worker, the corrective actions tak~n to*
address the apparent weaknesses identified and the adequacy of your QA/QC
program for the outage.
