Combined Inspection 50-272/88-02 and 50-311/88-02

This refers to the special team inspection of the December 22, 1987 flooding

of Service Water Bay Number 1, led by Mr. E. Kelly of this office, on January

4-7, 1988, at the Salem Nuclear Generating Station, Hancocks Bridge, New

Jersey.

The scope and findings were preliminarily discussed with Mr. J. M.

Zupko, Jr. and yourself at an exit meeting held on January 7, *1988.

Areas examined during this inspection are described in the inspection report

which is enclosed with this letter and include the design adequacy of service

water system isolation valves, the root cause and safety significance of the

flooding event, and status of service water Bay Number 1 restoration.

Interviews of personnel were conducted, service water equipment examinations

were made, and selected procedures and records were reviewed including an

assessment of your independent investigation of the event.

No violations of

regulatory requirements were identified during the inspection.

The inspection team identified several additional areas detailed in Detail 2.3

of the enclosed report, that warrant your further consideration and action.

These items include development of more specific guidance for setting valve

limit stops, identification of allowable leakage criteria and performance of

leak testing for service water isolation valves.

We understand that you will

address these items in a special report to be submitted to the NRC following

the results of analyses performed on the service water 13SW20 valve which failed.
