Combined Inspection Report Nos. 50-272/87-30; 50-311/87-31
A reactive safety inspection was conducted by Messrs. R. Loesch and A. Weadock
of this office on October 19-23, 1987, to review radiological protection
activities associated with three incidents.
The incidents included a spill
of contaminated water from a steam generator and subsequent decontamination
efforts, a potential exposure in excess of the regulatory limit (10 CFR
20.101) due to a fuel particle (hot particle), and the apparent defeating, on
two occasions during the same day, of a locked high radiation door.
All three
incidents were originally identified by your staff. The ~pisode involving the
locked high radiation area door is a continuing situation.
A similar
occurrence was identified in March, 1987 (Combined Inspection Report Nos.
272/87-07; 311/87-08).
Within the scope of this inspection, it appears that several of your
activities were not conducted in full compliance with regulatory requirements.
Three apparent violations are discussed in enclosure Appendix A.
You are
required to respond to these findings by following the instructions included
in Appendix A.
