This report is being submitted pursuant to the requirements of Technical Specification Action Statement 3.4.10.3.c within 30 days.

Mode 5 - Reactor Power Level 0% - Unit Load 0 MWe DESCRIPTION OF OCCURRENCE:
This Special Report describes a Pressurizer Overpressure Protection System (POPS) actuation.
The actuation occurred on August 23, 1987 at 2214 hours0.0256 days <br />0.615 hours <br />0.00366 weeks <br />8.42427e-4 months <br />.
This report is submitted for information in accordance with the requirements of Technical Specification Action Statement 3.4.10.3.c. which states:
In the event that either the POPS's or the RCS vents are used to mitigate an RCS pressure transient, a Special Report shall be prepared and submitted to the Commission pursuant to Specification 6.9.2 within thirty (30) days.
The report shall describe the clrcumstances initiating the transient, the effect of the POPS's or vents on the transient and any corrective action necessary to prevent recurrence.
On August 23, 1987, with the Unit in Mode 5 (Cold Shutdown), while the Board Nuclear Control Operator (NCO) was performing Reactor Coolant System (RCS) "Fill and Vent" operations, plant pressure increased to approximately 375 psig and Pressurizer Overpressure Protection Channel I (Power Operated Relief Valves 2PR1) actuated.
The POPS valve closed within a few seconds following actuation and the RCS pressure returned to approximately 325 psig.
APPARENT CAUSE OF OCCURRENCE:
The root cause of this event is personnel error.
The NCO did not initiate the corrective actions necessary to control plant pressure during the fill and vent operation.
NCO's are trained to recognize the close tolerance between the POPS actuation setpoint and RCS pressure during Fill and Vent operations.
A rise in pressure can occur which is sufficient to cause actuation of a POPS Channel.
ANALYSIS OF OCCURRENCE:
The Fill and Vent operations were performed in accordance with the
SPECIAL REPORT 87-4 ANALYSIS OF OCCURRENCE:
(cont'd) respective procedures.
Valve 2PR1 remained open for a few seconds.
Had the valve failed to close, the operator would have responded by closing blocking valve 2PR6.
All systems and indications functioned as designed.
There-was no undue risk to the health or safety of' the public due to this event.
In accordance with Technical Specification Action Statement 3.4.10.3.c, because the POPS was actuated to mitigate a RCS pressure transient, this Special Report is being submitted pursuant to the requirements of Technical Specification 6.9.2.
CORRECTIVE ACTION:
The Board NCO involved in this event was counseled.
Plant pressure control during fill and vent operations was stressed.
A letter to licensed operators from Operations management will be issued.
This letter will reiterate the-primary priority of licensed operators to assure continued safe plant operation at all times regardless of other activities or responsibilities.
A copy of this Special Report will be attached.
A review of the Operations Department Fill and Vent procedures shows that sufficient operator guidance is given.
No procedure modifications are required.
This event will be reviewed by the Nuclear Training Department for inclusion in applicable training programs.
