Reactor Vessel Sump Room Entries at Salem Station During the recent radiation protection inspection 86-13, our inspector was briefed regarding a reactor vessel sump high radiation area entry incident during the Salem 1 refueling outage.

No significant personnel exposure occurred and the response to the incident appeared adequate.

A review by regional staff has determined that an identical incident occurred at Salem 1 on October 10, 1980 (See Inspection Report No. 50-272/80-28).

In this earlier event, there was significant exposure to two personnel resulting in escalated enforcement action.

The NRC has been concerned for some time about control of personnel entries into the reactor cavity while the incore instrumentation thimbles are with-drawn creating extremely high radiation levels.

Guidance to licensees has been provided in IE Information Notices 76-03, 82-51 and 84-19.

Although the two incidents at Salem are widely separated in time and the earlier incident did not involve current management personnel, the similarity cannot be ignored.

You are requested to incorporate the 1980 event and the Information Nati ces into your review of the recent reactor sump entry and determine if additional action is appropriate, particularly in regards to the control afforded by the shift supervisors.

We request a reply on this matter within 90 days of the receipt of this letter.
