APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Salem Units 1 and 2 Docket Nos.
50-272, 50-311 License Nos. DPR-70, DPR-75 On April 12, 1985 an on-the-spot change was made to the Waste Gas Decay Tank Sampling procedure which changed the intent of the procedure and caused the samples to be meaningless.
Your entire on-the-spot change review process failed to identify the procedural deficiency and this resulted in 34 separate waste gas releases from April 12 to July 1, 1985, being made in vio-lation of Technical Specifications sampling requirements.
Although this vio-lation was licensee identified and we have verified that no hazard to the public existed, your use of the on-the-spot change system continues to be of significant concern to the NRC.
We understand that your recent reorganization may have an impact on your review process and your response to this violation should address this point.
On July 7, 1985, during the startup of the Unit 2, a reactor trip was caused by High High Steam Generator Water Level, which was a direct result of an operators failure to follow the procedure that gives the proper steps when shifting from manual to automatic feedwater control.
As a result of the inspection conducted July 1-31, 1985 and in accordance with the NRC Enforcement Policy (10 CFR 2, Appendix C) published in the Federal Register on March 8, 1984 (47 FR 8583), the following violations were identified:

Technical Specification 6.8.3 requires that written procedures established, implemented and maintained in accordance with Technical Specification 6.8.l may be changed under certain conditions.

Station Administrative Procedure AP-3, 11 Document Control Program" states that on-the-spot changes shall not alter the intent of the instruction/
procedure.
Contrary to the above, Chemistry Instruction CH-3.5.012, 11Gas Decay Tanks
- Sampling" was not adequately maintained in that on April 12, 1985, on-the-spot change number 1 was made to the instruction which changed the intent of the procedure and caused it to be inconsistent with the Tech-nical Specifications.
This is a Severity Level IV violation (Supplement I) applicable to DPR-70 and DPR-75.
Technical Specification 6.8.l requires that written procedures be established, implemented and maintained.

OP 15 11 Use of Operations Department Procedures, 11 states that the operator is responsible for performing evolutions properly and in the correct sequence.
Contrary to the above, on July 7, 1985, the operator did not utilize pro-cedure OP III.9.3.2 11 Feed Pump Operation, 11 and manipulated the controls for shifting feedwater controls in the wrong sequence, causing Unit 2 to trip on High.High Leve 1 in a steam generator.
This is a Severity Level IV violation (Supplement I) applicable to DPR-75.
Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter transmitting this Notice, a written statement of explanation in reply, including for each violation:
(1) the corrective steps which have been taken and the results achieved; (2) the corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Where good cause is shown, consideration will be given to extending the response time.
