Dear Mr. Uderitz:

As discussed in the meeting, you will provide:

1.

A chronological description of the events concerning the annunciator problem that took place on May 27, May 28, and May 30, 1983.

2.

Your assessment of the cause, or causes, of the problems.

Corrective actions you have taken and intend to take, both short-term and long-term.

Of particular interest is your plan to review these problems with the original supplier of the annunciator circuitry and the results of these findings and the remedial actions proposed.
