Several actions were initiated as a result of this meetinq and -

associated recommendations that were made by your managenent consultants.

We are interested in verifying the status of completion of these vario11s activities Jor~S~glem.Units 1 and 2.

Pursuant to 1 O CFR 50. 54( f), we request that you provide the \\l.IRC by April 25~ 1983, answers to the questions in the enclosure to this letter.

This information should be submitted to the NRC, signed under oath and affirmationa to enable the Commission to determine whether or not the license should be modified, s~.ispended, or revoked.

v' ENCLOSURE TO 10 CFR 50.54(f) LETTER PUBLIC SERVICE ELECTRIC AND GAS COMPANY

1.

As you may know, the NRC staff informed the Commission on April 13, 1'983, based on informati'on from your_ pl ant personnel, that both undervoltage attachments that fai~ed-o-n Salem Unit 1 on February 22 and 25 were in the possesston of Westinghouse on April 13.

Please verify whether that information is correct. ff not, please identify the current location of these failed attachments.

2.

Please advise us what maintenance, modfftcations or tests have been conducted since original purchase on the fai'led UV attachment that Westinghouse provided to Frankl i'n Institute, the NRC'-s contractor, subsequent to April 13,' 1983.

3.

Ohta in confi-rmation in writing from the reactor trip breaker vendor that the installed breakers are sattsfactory for plant operation and tha:t the Sa 1 em Ma i'rite:nance Procedure M3Q-2 wt 11 pro vi de the necessary basis to assure continuing operattonal re1~abi1fty.

4.

Obtain reports from the Statton_-Operattons ~evtew Committee and the Nuclear Review Board documenttrig-review--of sfiort-term aspects of trip breaker fatlures and concur-rens:e w-fth_ restart.

5.

Please explain why one would ~xpectto fi-nd dust and dirt in the breakers subsequent to _"the ma in"tert"ance-that was reportedly conducted in January 1983.

6.

Please describe your program for determfni'ng lffe cycle testing criteria including the breaker sampl tng techntque that wi*11 be employed.

Include a d'iscusston of how* the breaker testtng results will be used to develop a tri*p breaker replacement program.

7.

Please destribe those changes you have made or plan to make to ensure management is accountable for safe s-ftutdown of the Sal em facil Hies and will pursue an aggres:sive role in the_ s_afety of plant operation.

8.

Please provide the reactor trip 'data for each Salem Untt since January l, 1979.

Include all reactor trips, the initiation signal, and pre-trip power level.

9.

Please verify that the MEL now includes all items important to safety.

10.

Verify that the use of non-safety grade instrumentation to veri-fv the presence of a reactor trip demand signal will not degrade the safe opera~1on of the pl ant.

11. Please describe your basis for concluding that the additional 35 erroneously classified Work Orders that were found in your review had not adverse safety impact.
12.

Please identify any errors, omissions, or inaccuracies in the Draft Staff SER, Order, and Commission Briefing of. April 14, 1983.

13.

Verify that the detail listing of the history of the various locations of each of the 8 reactor trip breakers on Salem Units 1 and 2 is as stated in the NRC staff's evaluation as attached, and amend as necessary particularly filling in the entries noted by 11 ?u.
