I.

Description of Change.

Technical Specification Table 4.4-2 See Attachment I for description of change.

II.

Reason for Change The Technical Specification is to be changed to provide an alternate sampling method when the number of degraded or defective tubes detected is sufficient to require an additional inspection sample and the initial inspections indicate the degradation to be confined to a specific area of the tube sheet array or portion of the tube.

III. Safety Evaluation

Since the degradation is identified as being confined to a limited region, inspection of tubes outsjde the area of interest does not provide sufficient increase in information or safety to justify the added exposure of personnel to radiation associated with the inspection.

Substantial additional sampling was performed beyond the area of interest without any further evidence of degradation.

Westinghouse has advised *that *the nature of indications detected on the Salem.Unit No. 1 steam generators during.the third refueling o~tage is the same or similar to indications observed at Prairie Island Unit No. 2 which have been confined to the peripheral tubes.

Page 2 of 2 Alternate Action During the third refueling outage surveilance examinations, indications associated with wall reduction were detected on the tubing of No. 12 and No. 14 Steam Generators.

The con-dition was established as occurring on the outside diameter of tubes located around the periphery of the tube sheet on the cold leg side at the intersections of the first, sec-ond, and third support plates.

The following alternate action may be taken in pl~ce of that required by Table 4.4-2 whe~ the results of the ini-tial sample require that an additional sample or samples must be inspected and the condition for which the added inspection is required is limited to the peripheral tubes:

-The second inspection sample shall, as a minimum, include* an area of 5 tubes inward, measured from

  • the innermost defective or degraded tube and in an arc. completely around that side of the steam generator (cold leg or hot leg) where the initial indications-are found.
2.

Subsequent samples shall be taken in a simirar manner if degraded or defective tubes are found in the second sample inspection.

ATTACHMENT 2 PAGE 1 OF 2 SUPPORTING DOCUMENTATION During the third refueling outage of Salem Unit No. 1, indications of tube wall reduction were detected by multifrequency eddy current examination.

These indications were confined to-the peripheral tubes at the first, second, and third tube support plates in the cold leg side of the generator.

The inwardmost indications are located not more than 5 rows in from the edge of the tube sheet.

See Sketch No. 1 and Sketch No. 2.

Indications were first observed in the peripheral tubes of No. 12 Stearn Genera~or during the initial sample inspection.

A total of 539 tubes were initially scheduled

~o be inspected in No. 12 Stearn Generator to fulfill the Technical Specification requirement of 408 tubes (3% of the total number of tubes in four steam generators or 12% of the 3388 tubes in one steam generator) and to provide adequate coverage over areas of interest recommended by Westinghouse Corporation (see Sketqh #3).

Some tube inspection was scheduled for No. 14 Steam Generator but for information only an~ not as part.of the Technical Specification

  • requiremen.t.

When degraded tubes were detected in the first sample inspection of No. 12 Steam Generator, a change was made in the examination plan to include No. 14 Steam Generator in the Technical Specification sample program as defined in Table 4.4-2 (see Attachment 1, page 1) rather than concentrate the expanded sample required by the table in a single steam generator.

This change also changed the minimium sample size designator(S) from 12% to 6% (208 tubes) for each steam generator inspected according to the

-~ormula giv~n in the table.

Completion of the initial 539 tube sample in No~ 12 Steam Generator indicated 3 tubes defective,.1 tube degraded and 1 tube with a minor indication, all in peripheral tubes.

This placed Noe 12 Steam Generator into

.Category C-2 and a second sample of 408 tubes (2 S or 12%)

was inspected.

This sample concentrated on the periphery and was conducted from the cold leg side.

No additional defective or degraded tubes were found~

An initial sample of 229 tubes was then inspected in No. 14 Steam Generator, from the cold leg side.

These tubes were selected primarily from the periphery of the tube sheet but a grid pattern was also selected from interior tubes (See Sketch No. 4).

The first sample inspection indicated 2 tubes defective and 11 tubes degraded, all peripheral.

This placed ~o. 14 Steam Generator into Category C-2 also, and a second sample of 408 tubes was inspected.

The second inspection indicated an additional 6 tubes defective and 13 tubes degraded in the periphery,.placing No. 14 Steam Generator into Category C-3.

Inspection of the remaining tubes in No. 14 Steam Generator was then initiated, as required in Category C-3.

After inspecting 916 additional tubes in a wide arc around the periphery no evidence of further degradation was observed and the inspection was discontinued pending relief from the NRC.

As a result of the findings above, inspections have been scheduled for No. 11 and No. 13 Stearn Generators to be completed during the third refueling outage.

These inspections will include 408 tubes selected from.the periphery of each of the two steam generators.

Inspection

  • will be conducted from the cold leg side using rnuitifrequency eddy current method.

Should defective or degraded tubes be.found during these inspections, additional samples will be selected in accordance with the Alternate Action proposed in the License Change Request.

Present plans* call for" one of the defective tubes in-No. 14 Steam Generator (Row 44 Column 62) to be pulled for further evaluation.

As an added precaution the plugging limit has been lowered to 30% in No. 14 *steam Generator.
