Combined Inspection Number 50-272/81-03; 50-311/81-04
This refers to the routine safety inspection conducted by Mr. G. Napuda of
this office on February 9-13 and 18, 1981 at Salem Nuclear Generating Station
Units 1 and 2, Hancocks Bridge, New Jersey and your Newark, New Jersey
corporate offices of activities authorized by NRC License Nos. DPR-70 and
DPR-75 and to the discussions of our findings held by Mr. G. Napuda with
Messrs. J. Boettger, R. Uderitz and other members of your staff on February
13, 1981; and Messrs. H. Midura, F. Myer and other members of your staff on
February 18, 1981 and to a subsequent discussion during a meeting between our
Mr. L. Norrholm and your Mr. H. Midura on March 2, 1981.
Areas examined during this inspection are described in the Office of
Inspection and Enforcement Inspection Report which is enclosed with this
letter. Within these areas, the inspection consisted of selective
examinations of procedures and representative records, interviews with
personnel, and observations by the inspector.
Based on the results of this inspection, it appears that one of your
activities was not conducted in full compliance with NRC requirements, as set
forth in the Notice of Violation, enclosed herewith as Appendix A.
This item
of noncompliance has been categorized into the levels described in the Federal
Register Notice (45 FR 66754) dated October 7, 1980.
You are required to
respond. to this letter and in preparing your r;=;~~:tnse, you should follow the
instructions in Appendix A.

Our inspector also verified the steps you have taken to correct the items of
noncompliance brought to your attention in letters dated May 6, 1980 and
September 29, 1980.
We have no further questions regarding the steps you took
to correct Items A. l, B. l and 3, and C (May 6 .. letter) or Items A and B
(September 29 letter).
Item A in the Notice of Violation enclosed with this letter includes two items
which were identified during a previous inspection of your licensed activities
on February 1, 4-7, and 11-13, 1980, and were documented as Items A.2 and B.2
in the enclosure to our letter dated May 6, 1980.
Your letter to this office
dated June 3, 1980 stated corrective action would be fully implemented by
September 30, 1980 for these items.
From our February 9-13 and 18, 1981
inspection it appears that the stated corrective actions were: (1) not
implemented since one portion of Item A.2 was not corrected until November 3,
1980 and the other portion remains uncorrected; and, (2) not effective since
Item B.2 has recurred.
Recurrent and uncorrected items of noncompliance are given additional weight
in the consideration and selection of appropriate enforcement action.
Therefore, in your response to this letter, you should give particular
attention to those actions taken or planned to ensure that identified items of
noncompliance will be completely corrected and will not recur.

Relative to the individual items cited in the Notice of Violation, we
recognize that each item taken by itself may not have safety impact; however,
the pervasiveness of the sum total indicate an area where preventive and
improved management controls can preclude future safety problems.
Therefore,
in your response, we request that you focus attention on your
corrective action programs, specifically the actions that you have taken and
intend to take to improve their comprehensiveness and thoroughness.
In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of
this letter and the enclosures will be placed in the NRC 1 s Public Document
Room.
If this report contains any information that you (or your contractors)
believe to be exempt from disclosure under 10 CFR 9.5(a), it is necessary that
you (a) notify this office by telephone within ten (10) days from the date of
this letter of your intention to file a request for withholding; and (b)
submit within 25 days from the date of this letter a written application to
this office to withhold such information.
Consistent with section
2.790(b)(l), any such application must be accompanied by an affidavit executed
by the owner of the information which identifies the document or part sought
to be withheld, and which contains a full statement of the reasons on the
basis which it is claimed that the information should be withheld from public
disclosure.
This section further requires the statement to address with
specificity the. considerations listed in 10 CFR 2.790(b)(4).
The information
sought to be withheld shall be incorprated as far as possible into a separate
part of the affidavit.
If we do not hear from you in this regard within the
specified periods noted above, the report will be placed in the Public

The telephone notification of your intent to request
withholding, or any request for an extension of the 10 day period which you
believe necessary, should be made to the Supervisor, Files, Mail and Records,
USNRC Region I, at (215) 337-5223.
