COMBINED NRC INSPECTION 50-272/81-05 AND 50-311/81~06 SALEM GENERATING STATION
-UNITS NOS. 1 AND 2 We have reviewed the report of your inspection conducted by Mr. L.
J. Norrholrn during the period March 1-31, 1981.
Our response to the item of noncompliance identified in Appendix A is as follows:
Item of Violation Technical Specification 6.8.l states, in part, "Written procedures shall be established, implemented and maintained covering... the applicable procedures recommended in Appendix A of Regulatory Guide 1.33, November 1972."
Regulatory Guide 1.33 Appendix A includes Paragraphs C.10 Main Stearn Systems, C.14 Chemical and Volume Control System, C.18 Instrument Air System,.and G.3 a PWR Gas System (Collection, Storage, and Discharge).
Contrary to the above, the following failures to provide procedures for portions of the above listed systems were identified:
- a.
Intermittent use of valve 1CV243 to control Volume Control Tank pressure by venting to the vent header was not addressed by an approved operating procedure on March 11, 1981.

- b.
Operation of valves 1DR97 and 1DR98 to provide and maintain seal water on the waste gas compressor seals was not addressed in an approved operating procedure on March 11, 1981.
- c.
Operation of air supply valves to the redundant air supply for Main Steam Isolation valves was not addressed in an approved procedure on February 11, 1981 resulting in an aµtomatic plant trip due to a closed valve.
Reply to Violation A.
The actions for a Volume Control Tank Hi-Low Pressure alarm on the control console did not specify how to return pressure to normal.
An on~the-spot change has been made to incorporate the use of lCV 243 to vent the VCT to return pressure to normal.
This change will be added to the control console alarm list during the next review/revision cycle.
We are in compliance now.
B.
The procedure for Demineralized Water -
Normal Operation did not include valves lDR 97 and 1DR98 in the system valve line-up sheet. _ The noted valves have been added to Operating Instruction III-15.3.l with an on-the-spot change.
An evaluation of the Gaseous Waste Disposal System Procedure for additional precautions for abnormal system conditions resulted with no procedural changes.
Any temporary system operations will be performed in accordance with procedure changes made by an on-the-spot change.
We are in compliance now.
- c.
At the completion of a design change package test the redundant air supply valve was inadvertently left closed which caused the main steam stop valve to close on a control air supply transient.
The subject valve was reopened and the other redundant air supply valves installed on the same DCP were verified open.
To prevent recurrence a design change request has been initiated to assign generation numbers to redundant air supply valves to the air supply panels.
The numbers will then be incorporated into the appropriate Operating Instruction's valve lineup sheets.
The projected date for completion is October 1, 1981.
