This refers to the telephone conversation between you and Mr. George Smith of our office on October 24, 1980 and discussions between Mr. R. L. Nimitz of this office and Mr. H. J. Midura, Manager - Salem Generating Station, on October 17, 1980, regarding organizational interactions during activities having the potential for substantial personnel exposure such as the recent entries into the fuel transfer tube and into the cavity under the reactor vessel at Salem Unit 1.

With regard to the matters discussed, we understand that you have undertaken or will underake the following actions:

1.

All site supervisory personnel have been instructed to take all necessary action to prevent inadvertent or unnecessary radiation exposure of personnel due to tasks under their direction.

By October 31, Senior Shift Supervisors will be instructed in their special responsibility in this area.

2.

The reactor flux thimble maintenance procedure has been revised to require that the Senior Radiation Protection Supervisor be notified and the Senior Shift Supervisor has verified that access to the thimble area has been locked and tagged, before thimble retraction occurs.

3.

The radiation exposure permit procedure will be revised by October 24 to prohibit work not described on the permit and to designate individuals authorized to give radiation protection approval.

4.

A special meeting of the Station Operations Review Committee was held on

  • October 16 to review the fue 1 transfer tube and reactor cavity entries. A series of additional corrective actions were recommended.

All recommendations, excluding training of plant supervisors, will be completed by November 28, 1980. This training will be implemented before the next refueling outage.
