Mr. John J. Mattimoe Assistant General Manager and Chief Engineer Gentlemen:

This letter relates to the testing of concrete expansion anchor bolts used in pipe support base plates as discussed by telephone between Mr. R. J. Rodriguez and others of your staff and Mr. B. H. Faulkenberry and other members of this office on January 10 and 14, 1980.

Based on this discussion, it is our understanding that, prior to resuming reactor power operation after completion of your current refueling outage, you are to take the following actirins:

l. Complete the actions required by IE Bulletin 79-02_, dated March 8, 1979, and its revisions dated June 21,-:i~97.9:=a-n-d~Novem6er 8, 1979.
2.

Complete the testing of 100% of all accessible concrete expansion anchor bolts installed in all Category 1 systems located outside containment and correct all deficiencies found during the performance of the testing.

3.

Complete the testing of 100% of all accessible concrete expansion anchor bolts installed in all Category l systems located within containment and correct a 11 de.fi c i enc i es found during the performance of this testing, or through the program of testing described in IE Bulletin 79-02 and its rev1s1ons verify at the 95% confidence level that there are less than 5%

defective anchors installed in any one Category l system.

4.

Perform inspections to the extent possible on those anchors determined to be inaccessible and correct all deficiencies identified. These inspections, as a minimum, should include visual inspections for presence and size.
