APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-272 Based on the results of an NRC inspection conducted on April 22 - May 26, 1979, it appears that certain of *your activities were not conducted in full compliance with the Code of Federal Regulations and the conditions of your NRC Facility License No. DPR-70 as indicated below.

Item A is an Infraction, Item B is a Deficiency.

A.

Technica 1 Specification 4. 7.10.4 ~tates, in part, "Each of the fire hose stations shown in Table 3.7-11 shall be demonstrated OPERABLE:

a.

At least once.per 31 days by visual inspection of the station to assure all required equipment is at the station.

  • Contrary to the above, on May 10, 1979, no evidence could be identified to demonstrate that the above two fire hose stations had been inspected since March 21, 1979, a period exceeding 39 days (31 days plus 25%).

B.

Facility Operating License DPR-70, Appendix A and Appendix B, Technical Specifications detail the following requirements for event reports to the NRC:

116.9.l.8 The types of events listed below shall be reported within 24 hours2.777778e-4 days <br />0.00667 hours <br />3.968254e-5 weeks <br />9.132e-6 months <br /> by telephone and confirmed by telegraph, rnailgram, or facsimile transmission to the Director of the Regional Office, or his designate no later than the first working day following the event, with a written followup report within 14 days...

Operation of the unit or affected systems when any parameter or operation subject to a limiting condition for operation is less conser-vative than the least conservative aspect of the limiting condition for operation established in the technical specifications.

116.9.1.9 The types of events listed below shall be the subject of written reports to the Director of the Regional Office within thirty days of occurrence of the event...

b.

Conditions leading to operation in a degraded mode pennitted by a limiting condition for operation or plant shutdown required by a limiting condition for operation.

11 115.6.2.l Nonroutine Environmental Operating Reports A report shall be submitted in the event that (~) a limiting condition for operation is exceeded (as specified in Section 2.0, 11Limiting Conditions for Operation 11 ), {b) a report level is reached (as specified in Section 3.0, 11 Environmental Surveillance 11 ), or (c) an unusual or important event occurs that causes a significant*environmental impact, that affects potential environmental impact rrom plant operation, or that has high public or potential public interest concerning environmental impact from plant opera-tion.

  • Reports shall be submitted under one of the report schedules described below.

  • 30'-Day Report.* Those events not requiring a prompt report shall be reported within 30 days by a written report to the Director of the Regional Inspection and Enforcement Office (with copy to the Director, Office of Nu~lear Reactor Regulation.)
