The purpose ~f this letter i~ to request additional information which we will need as a basis for evaluating your request for an extension.

Please provide us with the following additional

Justification for 'extending the 1 a test date for completion of a Construction Permit must be a show of' good cause based upon events or circumstances beyond the control of the applicant which have or 0ill occur subsequent to the issuance of the Op~rating License.
