APPENDIX A NOTICE. OF VIOLATION Public Service Electric and Gas Company Docket No. 50-272 Based. on the resu.l ts of an NRC inspection conducted* February 13-16, 1979, it appears that one of your activities was not conducted in full compliance with the conditions of.your NRC Facility License No. DPR-70 as indicated. below.. This item is a deficiency.
Technical Specification 6.8.1 states,_ in part: "Written procedures shall be *.* implemented... covering... surveillance and test activities of safety related equipment...

- surveillance Procedure SP(O) 4.2.1.1, Power Distribution - Axial Flu~ Difference (AFD), Unit 1, Revision 2, March 10, 1978, requires, in part, that data be recorded on check~off sheets 4.1. 4.2, or 4.3 and that usage of specific check-off sheets be. indicated as appropriate on the procedures cover page by initials. Check-off sheet 4.1. AFD Data, requires, in part,. the use of target values in the current copy of the associated data sheet. from.the Reactor Engineering Manual.
Check-off 4.2, AFD Alarm Inoperable requires, in* part, the computation of deviation between actual AFD value and target AFD value and associated review signatures by the operator and Shift Supervi~or. Check-off 4.3, Axial Flux Deviation Log, requires, in part, the. recording of 11 Power Range Channels Outside Target Band.
11 Contrary to the above, SP(O) 4.2.1.1 was not. implemented from August 28, 1978 to February 16, 1979 as noted be 1 ow:
the current target axial difference had not been used in the performance of SP ( 0) 4. 2. 1. 1 check-off 4. 1 from October z*,
- 1978 to February 16, 1979.
In twelve of thirty-seven instances,. the cover sheet of SP(O) 4.2.1.1 was either not used or appropriate check-off sheet usage was not marked.
On September 6, 1978, the required computation of deviation between target and actual AFD values and required signatures per check-off sheet 4. 2 were not completed;. and on September 3, 1978 the power range channel outside target band were not recorded on. check-off 4.3 despite the accumulation of penalty minutes (implying at least two channels were outside target band).
