NRC INSPECTION REPORT 50-255/97002(0RP)
Dear Mr. Palmisano:

On March 3, 1997, the NRC completed an in.spection at your Palisades Nuclear Generating
Plant. The enclosed report presents the results* of that inspection.
The inspection covered a 7-week period. Areas examined during the inspection are
- identified in the report. Within these areas, the inspection consisted of a selective
examination of procedures and representative records, interviews with personnel, and
observation of activities in progress. The purpose *of the inspection effort was to .
. determine whether activities authorized by the license were conducted safely and in
accordance with NRC requirements'. The summary contained in the report provides an
overview of our asse~sment ,of your performance.
Three forced outages occurred during this inspection period. *Analysis of each of the
outages determined that they were caused by rework or plant material condition problems.
These outages resulted from recurring steam leaks on both main steam isolation valves,
recurring degradation of the main generator isophase flex links and a hydrogen leak on the
main generator hydrogen seal oil system. R*ework, plant material condition and timely
- corrective action were recurring themes from the previous .inspection report .. Although
material condition and rework continued to challenge operators, operator response to these
conditions .was found to be good. Based on concerns expressed by the inspectors, reactor
engineering was stationed onshift to provide appropriate monitoring and oversight of *radial
peaking factors. In addition; aggressive action was taken in response to weaknesses in
monitoring and oversight of minimum temperature for criticality, which included providing
"just-in-time" simulator. training to the operating crew .
