SUBJECT:

NRC INTEGRATED INSPECTION REPORT NO. 50-255/96014(DRP)

AND NOTICE OF VIOLATION

Dear Mr. Palmisano:

On November 23, 1996, the NRC completed an inspection at your Palisades

Nuclear Generating Plant. The enclosed report presents the results of that

inspection.

Based on the results of this inspection, the NRC has determined that your

plant was operated in a safe manner.

We are concerned, however, about two

violations of NRC requirements that were identified. The first violation is

of concern because it includes two examples where your licensed operating

staff failed to adhere to procedures while performing outage related

.activities. Inadequate coordination and conununication between work groups

performing the outage activities contributed to this violation. The second

violation is of concern because of your staff's failure to provide adequate

installation instructions for a temporary modification on the containment

polar crane.

These violations are cited in the enclosed Notice of Violation (Notice), and

the circumstances surrounding the violations are described in detail in the

enclosed report. Please note that you are required to respond to this letter

and should *follow the instructions specified in the enclosed Notice when

preparing your response. * The NRC will use your response, in part, to

determine whether further enforcement action is necessary to ensure compliance

with regulatory requirements.
