PALISADES INSPECTION REPORT 50-255/99004(DRP)
Dear Mr. Palmisano:
On May 21, 1999, the NRC completed an inspection conducted at your Palisades Nuclear
Generating Plant. The enclosed report presents the results of that inspection.
The inspection covered a 6-week period. Areas examined during the inspection are identified in
the* report. Within these areas, the inspection consisted of a selective examination of
procedures and representative records, interviews with personnel, and observations of activities
in progress. The purpose of the inspection effort was to determine whether activitiE!S authorized
by the license were conducted safely and in accordance with NRC requirements.
During this inspection period, we observed that the plant was operated in a conservative
manner and that effective controls were*implemented during potentially high risk evolutions
which demonstrated a positive focus on safety. Your decis'ion to place the plant in a cold
- .shutdown condition to repair the Primary Coolant Pump P-50A seal package was pro-active and
conservative. Also, radiation dose received during the outage was significantly less than*
projected because of radiological personne,l's efforts to create a heightened awareness related
to work activities in radiation areas.
However, a maintenance outage was required, in part, to repair an oil leak on a Primary
Coolant Pump. The leak was subsequently determined to be a maintenance preventable
functional failure. Primary Coolant Pump oil leaks have been a recurring problem and past
repairs have not been completely effective. Also, a Main Steam Isolation Valve failed to fully
close during required testing which was likewise considered a maintenance preventable
functional f.ailure. Both of these issues were recurring problems from the December 1998.
o'utage. These issues did not result in any adverse safety consequences; however, further.
attention to resolve recurring equipment problems is warranted.
No violations or deviations of NRC requirements were identified.
