NOTICE OF VIOLATION (NRC INSPECTION REPORT NO. 50-255/98009(DRS))
Dear Mr. Palmisano:
This will acknowledge receipt of your letter dated July 14, 1998, in response to our letter
dated June 15, 1998, transmitting a Notice of Violation associated with the three radiation
protecti~n violations which were identified during your 1998 refueling o~tage. These violations
were as follows: (1) the failure to properly post the reactor cavity as a high radiation area, (2) the
failure to exert positive control over radiation controlled doors on two separate occasions, and
(3) two examples for failure to cainply with the radiation controlled area access procedure during
work in the reactor cavity. We also acknowledge your clarification of the contamination levels
detected as a result of poor radworker practice. We have reviewed your corrective actions and
have no further questions at this time. These* corrective actions will be examined during future
inspections.
