NOTICE OF VIOLATION AND NRC INSPECTION REPORT
NO. 50-255/98002(DRP)
Dear Mr. Palmisano:
On March 13, 1998, the NRC completed an inspection at your Palisades Nuclear Generating
Plant. The enclosed report presents the results of that inspection.
The inspection covered a seven-week period. Areas examined during the inspection are
identified in the report. Within these areas, the inspection consisted of a selective examination of
procedures and representative records, interviews with personnel, and observation of activities in
progress. The purpose of the inspection effort was to determine whether activities authorized by
the licensee were conducted safely and in accordance with NRC requirements. The summary
contained in the report provides an overview of our assessment of your performance.
The inspectors noted that management's decisions regarding plant maneuvering during plant
startup following the planned outage were conservative. Also, the inspectors noted that the
reactivity manipulations conducted during the plant startup were controlled and deliberate.
However, some weaknesses in crew communications and command and control were observed
in the control *room.
Based on the results of this inspection, the NRC has determined that a violation of NRC
requirements regarding procedure adherence in the control of watertight doors occurred. The
violation is cited in the enclosed Notice of Violation (Notice), and the circumstances surrounding
it are described in the enclosed report. This violation is of concern because of the potential loss
for two trains of safeguards equipment which your staff failed to recognize.

You are required to respond to this letter and should follow the instructions specified in the
enclosed Notice when preparing your response. The NRC will use your response, in part, to
determine whether further enforcement action is necessary to ensure compliance with regulatory
requirements .

In addition, the inspectors identified that a large backlog of outstanding procedure change
requests existed and that the backlog was not prioritized. We were concerned that the failure to
prioritize the large backlog could preclude timely completion of requested procedure changes.
Also, this accumulation of procedure changes could have a negative impact in that personnel
who are aware of the backlog may be reluctant to submit additional needed procedure change
requests. We understand that your corrective actions included a review and prioritization of the
procedure change backlog and allocation of additional resources to the procedures group.
In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of this letter, the
enclosures, and your response will be placed in the NRC Public Document Room.
