NOTICE OF VIOLATION AND NRC INSPECTION REPORT

NO. 50-255/97013(DRP)

Dear Mr. Palmisano:

On December 5, 1997, the NRC completed an inspection at your Palisades Nuclear Generating

Plant. The enclosed report presents the results of that inspection.

The inspection covered a seven-week period. Areas examined during the inspection are

identified in the report. Within these areas, the inspection consisted of a selective examination of

procedures and representative records, interviews with personnel, and observation of activities in

progress. The purpose of the inspection effort was to determine whether activities authorjzed by

the license were conducted safely and in accordance with NRC requirements.

Based on the results of this inspection, the NRC has determined that a violation of NRC

requirements occurred. The violation is cited in the enclosed Notice of Violation (Notice), and the

circumstances surrounding it are described in detail in the enclosed report. The violation is of

concern because an inadequate procedure allowed operators to start a primary coolant pum_p

without meeting Technical Specification requirements. Of additional concern are the Technical

~pecification aspects of this event and the lack of a questioning attitude by the operators

involved. Recently, a similar Violation (50-255/97008-01 (DRP)) was issued for a procedure

which did not adequately prevent operation of the reactor above the licensed thermal power

limits. In the earlier case, the Technical Specifications were not violated; however, in this event,

the operators started primary coolant pump P-50D even though the procedure required the

operators to refer to the appropriate Technical Specification.

You are required to respond to this letter and should follow the.instructions specified in the

enclosed Notice when preparing your response. The NRC will use your response, in part, to

determine whether further NRC enforcement action is necessary to ensure compliance with NRC

regulatory requirements.
