- concerning the Palisades Third Interval lnservice Inspection Program submitted on September 6, 1995. This letter provides that information.
Attachment 1 contains copies of the NRC request items and our response to each item.
Attachments 2, 3, and 4 contain the revised schedule pages for Sections 6, 8, and 9 of the Third Interval Program relating to the reactor vessel weld examinations after annealing. Sections 6 and 8 are replaced in entirety for convenience. For Section 9, only the Population Summary and the first 1, 3 pages of the Interval 3 Program Plan are attached since they are the only portions affected by the schedule change. Attachment 5 contains a revised Relief Request RR-4. Attachment 6 contains the Palisades Pressure*Testing Program EM-09-13, which is being submitted as requested.

RESPONSE TO REQUEST FOR ADDITIONAL INFORMATION FOR INSERVICE INSPECTION PROGRAM NRC REQUESTED INFORMATION (1) In the December 20, 1995, RAJ, you were requested to verify that at least 90% of each reactor pressure vessel (RPV) shell weld (Items B 1. 11 and B 1. 12) has been examined to satisfy augmented RPV examination requirements specified by the regulation in 10 CFR 50.55a(g)(6)(ii)(A)(2). In the February 19, 1996, response, you stated "Effectively, 100% of the accessible welds which are required for examination were examined by our contractor (Westinghouse) during the 1995 refueling outage." This response is not clear. Confirm that at least 90% of each of the Item B 1. 11 and B 1. 12 shell welds have been examined and provide the percentage of the Code-required volume that was examined for each of these welds.
CPCO RESPONSE (1) CPCO has reviewed the Westinghouse Reactor Vessel Examination report from the 1995 refueling outage to reconfirm that at least 90% of. the ASME Section XI required volume on the Reactor Pressure Vessel, Category 8-A, Item 81 .11 and 81 .12 were examined.
Identified below are the specific weld numbers and the examination percentages which were achieved for each weld:

2 (2) In the summary by category contained in the Program, a total of 44 areas are identified for augmented examination, but are not scheduled during the third 10-year interval. Is this an oversight? Provide a clarification regarding the scheduling of these areas.
CPCO RESPONSE (2) The 44 are*as identified for augmented examination are scheduled for the third 10-year interval. As stated in the Section 6 Introduction, the summary by category as referenced in the NRC requested information only identifies the periods where Section XI Code credit is being taken. ASME Section XI Code credit is not being taken for the 44 augmented examinations identified. This is why 0% is identified on the third interval summary by category.
Section 9 (Pages 1-4) of The Third Ten-Year Interval Inspection Program Plan contains the 44 Augmented Examinations, and also identifies the periods and outages in which they are scheduled.
NRC REQUESTED INFORMATION (3) All 27 of the RPV welds that are accessible are scheduled to be examined during the third inspection period in .accordance with Section XI. _However, in response to the RAJ question on Request for Relief RR-9, you stated that the_ examinations performed following the vessel anneal (at the end of the first period) would be used to satisfy the examination requirements for the vessel and attaching welds.
Will the RPV be examined following the vessel anneal, as well as, at the end of the interval? Provide clarification regarding the scheduling of examinations for the RPV welds.
CPCO RESPONSE (3) If the RPV is annealed at the end of the first period, the examinations performed will be used to satisfy the Section XI interval examination requirements for the vessel and attaching welds.
Therefore, CPCO has elected to withdraw Relief Request RR-9 at this time and will re-submit if the anneal is not performed. The revisions to the schedule for Sections 6, 8, and 9 are contained in Attachments 2, 3, and 4.
,. ** NRC REQUESTED INFORMATION (4) Request for Relief RR-9 requests to defer the examination of the RPV 3
nozzle-to-vessel welds and inside radius sections to the end of the third interval.
In the December 20, 1995, RAJ, you were asked about this relief and how it would be effected by a mid-interval vessel anneal. In response, you stated that the RPV anneal, currently scheduled for the 1998 refueling outage, would make this relief unnecessary. It was also stated that the fourth interval examinations would be performed at the end of the first period so that the time between exams did not exceed 10-years. This implies that the nozzle exams will not be deferred.

Provide a clarification regarding the status of this request for relief.
CPCO RESPONSE (4) CPCO has elected to withdraw Relief Request RR-9 at this time and will re-submit if the vessel anneal is not performed. The revisions to the schedule for Sections 6, 8, and 9 are contained in Attachments 2, 3, and 4.
NRC REQUESTED INFORMATION (5) Request for Relief No. RR-3 seeks relief from examining the steam generator shell welds to the extent required by the Code. However, the limiting factor causing a reduction in coverage (approximately 25%) is not clearly stated. In addition, coverage is limited as 75% and 98% in the axial directions and' 100% in each of the tWo circumferential directions . . Cumulatively, this* amounts to 93%
coverage and relief would not be required. Provide a descnption of the limiting condition for this weld and describe how examination coverage is determined at Palisades.
CPCO RESPONSE (5) The limiting factors causing a reduction in coverage are welded patches, snubber attachments, 18" feedwater nozzles and an abrupt transition (Configuration of the Shell Cone) which results in a loss of contact as the exit point of the transducer travels across this point. These limitations are shown on Combustion Engineering drawings 70277-371-021, 70277-221-001,
- 70277 "'201-001, and RSG90-C271-001 which were previously submitted (Palisades' Response to Request for Additional Information - lnservice Inspection Program dated February 19, 1996).
Examination coverage for welds at CPCO is based on the most limiting ultrasonic beam angle with two direction coverage. CPCO believes that this
4 conservative approach is the correct way to determine the percentage of examination coverage.
NRC REQUESTED INFORMATION (6) Request for Relief RR-4 requests relief from volumetrically examining the steam generator nozzle-to-shell welds to the extent required by the Code. It is obvious that complete examination from the nozzle side is impractical (if not impossible).
However, considering the configuration, the axial coverage from the nozzle side appears high. Provide an explanation describing how the axial scan from the nozzle side was performed.
CPCO RESPONSE *

- To determine the most accurate approximation for the area of coverage for this relief request, CPCO, working with Electric Power Research Institute (EPRI),
has developed a computer model of these welds. Based on the modeling which was performed, Relief Request RR-4 has been revised and is resubmitted in Attachment 5 for your review. The results from the EPRI computer modeling are ,
being formulated into a final report which will be available at Palisades upon receipt from EPRI.
- The steam generators were replaced during the middle of the second interval.
Review of the preservice data indicated that no exam was performed from the nozzle side. The computer modeling which *was performed accurately fdentifies the exam coverage~ an*d the optimum transducer angles to be used for these
- examinations.
- NRC REQUESTED INFORMATION (7) The revised version of Request for Relief RR-05 includes Items 83. 150 (nozzle-to-vessel welds) and 83. 160 (inside radius sections). However, the inside radius (JR) sections are not addressed. If relief is required for the JR sections, provide a description of the limitations, the estimated coverage, and any low coverage obtained(< 20%), provide a description of how reasonable assurance of the structural integrity will be provided for the nozzle-to-vessel welds.
CPCO RESPONSE (7) Relief is required forthe inside radius section as identified on Relief Request RR-5 for welds E-56A-05 and E-568-05. The inside radius sections, along with
5 the nozzle to vessel welds are limited due to the weld being covered by the support pads which make it inaccessible for any kind of a surface or volumetric examination within the area of the pads. In RR-5, the inner radius weld volume is considered as one of the 4 required weld volumes which was identified.
The design of welds E-56A-07 and E-568-07 do not have inside radius sections (Reference Atlas Industrial Drawings 1733-6 and 1759-4 which were supplied with Palisades' Response to Request for Additional Information - lnservice Inspection Program dated February 19, 1996).
As identified on the referenced drawings, there are two Class 1 nozzles on the Regenerative Heat Exchangers and are identified as nozzles "O" and "P". The subject welds (E-56A-07 and E-568-07) are the cross connect between the two vessels and are more of a "stub-in" configuration. Therefore, these two welds do not have an inner radius section and are not actually nozzles.
Based on the Regenerative Heat Exchangers being vessels (Category 8-D),
CPCO has identified the two welds, E-56A-07 and E-568-07, as Item Number 83.150 and scheduled these welds for examination. Item NumberB3.160 is not applicable for this application.
Reasonable assurance of the structural integrity will be provided for the welds identified in Relief Request RR-5 by performing examination on the accessible weld volumes along with the required VT-2 examinations which are performed each refueling outage.
The Regenerative Heat Exchangers are downstream of control valve CV-2001.
In the event of a weld failure (within these components) which creates an unidentified leak rate within the Primary Coolant System of greater than 1 gpm, Plant Technical Specification 3.1.5a requires placing the reactor in hot shutdown within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br />. If this were a small leak of less than 1 gpm Off Normal Procedure 23.1 would identify the charging to letdown mismatch and would close CV-2001 and isolate the leak. If one of these welds were to fail creating a large loss ,of coolant, Emergency Operating Procedure, EOP-4.0, "Loss of Coolant Accident Recovery", Section 4.0, Step 16 would also close CV-2001 isolating the leak.
The ultrasonic examinations, VT-2 examinations and the ability to correctly identify and isolate a leak from any one of these welds provides reasonable assurance that structural integrity will be maintained.
6 NRC REQUESTED INFORMATION (8) In response to an NRG RAJ question regarding containment penetrations, you referenced Code Case N-522, Pressure Testing of Containment Penetration Piping, and stated that a request to use this Code Case would be submitted with a new pressure testing submittal. It was also noted that there is a separate program (EM-09-13, lnservice Inspection Pressure Testing Program) that has not been included with the /SI program. Provide a copy of the pressure testing program so that it can be reviewed with the IS/ program.
CPCO RESPONSE (8) A submittal by CPCO to the NRC dated March 11, 1996, requested approval of Relief Requests PR-02 and PR-04. Relief Request PR-04 concerned alternative
. rules for the pressure testing of containment penetration piping in accordance with Code Case N-522. A copy of the submittal letter will be sent directly to the reviewer for information.
The Pressure Testing Program, EM-09-13, is included as Attachment 6 to this.

- 6. VERIFICATION OF SECTION XI COMPLIANCE A. INTRODUCTION The following tables document compliance. for the third interval. with the examination distribution requirements of Section XI of the ASME B&PV Code. The tables identify the total number of components by category which are subject to distribution and the total number of components for Inspection Program B.
B. DETERMINATION OF COMPLIANCE
- 1. First Period The minimum and maximum number of components required to be examined during the first period has been determined by applying the mi.nimum and maximum percentages cited in the code. Table(s) IWB-2412-1. IWC-2412-1 and IWD-241201 ..
The minimum number of components to be examined is 16% of the total.
components. The maximum number of components which can be examined is 34% of the total number of components.
- 2. Second Period The minimum number of components to be examined is 50% to the total components. The maximum number of components which can be examined is 67% of the total number of components.
The Section XI creditable percentage for this period is computed by dividing the number of components examined by the total number of components.
- 3. Third Period The minimum and maximum number of components to be examined is'obta1ned based solely on the number of examinations required to complete the cumulative total.
C. NUMBER OF COMPONENTS The number of components subject to examination per period will vary throughout the life of the plant due to code changes. relief requests and line walking.
Compliance per period is based solely on the number of components subject to the examination for that period. *

- D. INTERVAL COMPLIANCE Third period cumulative percentage totals which equal or exceed 100% verifies compliance with the distribution requirements of Section XI. A separate table verifies compliance for those examinations which are deferred to the end of the interval.
In the event that the number of examinations subject to distribution in a category decreases. and it is determined that it is impossible to achieve 100% without re-examining components. the component will not be reexamined. For example:
- 1. During the first period. 5 of 20 components in a category are examined (ie.
25%).
- 2. During the second period. it is determined that only 15 components actually exist in that category and that 5 additional components are examined (ie. 33-1/3% for this interval third and 58.3% cumulative).
- 3. The 5 remaining components are examined during the last period (ie. a cumulative total of 91.7%).
Under this situation. which could result from a number of reasons. no further examining is required provided adequate documentation substantiates the anomaly.

CODE REQUIREMENT Section XI, Table IW8-2500-1 requires all nozzle to vessel welds to be volumetrically examined once during each inspection interval.
BASIS FOR RELIEF
-For purposes of discussion, Figure IW8-2500-7(a) (attached) will be used to describe the 4 required weld volumes. With the exception of the nozzle inner radius section, this figure is the closest configuration to our actual nozzles.
- CPCO working with EPRI have developed the attached package. The attached information is an excerpt from the EPRI report and is intended to identify the exam volumes within the areas of Umitations. The final EPRI report will identify exam volumes within the area.~. of limitations (attached information), exam volumes where no limitations exiSt* and the composite exam volumes. The final composite exam volumes will be slightly higher since this will include the areas where no limitations exist.
However, the code required exam volume will not be achieved and this relief request is necessary.
The attached package includes:
- 1) Figure defining exam volumes.
- 2) Inlet nozzle inner radius coverage table followed by supporting figure.
- 3) Inlet nozzle-to-shell weld exam volume.
- 4) Axial scan coverage table for inlet nozzle-to-shell weld followed by supporting figures.

- 6) Transverse scan (+/-20° probe skewing) coverage table for inlet nozzle-to-shell weld followed by supporting figures.

- 8) Outlet nozzle-to-shell weld exam volume .

There are 2 acronyms used in the EPRI report for identification, they are Consumers Power - Steam Generator Project Inlet Nozzle (CP-SGPIN) and Consumers Power -
Steam Generator Project Outlet Nozzles (CP-SGPON).
The computer based modeling was performed on one steam generator and this is intended to address all primary head nozzle welds in both steam generators. The steam generators are identical in design.
The probe skew angle for the axial exams are identified as 0° and 180° within the coverage tables. The probe skew angle for the transverse exams were modeled using a 90° and then offset using a 70°and 110° skew to increase exam volume coverages.
In* summary, the examination volumes are limited and the maximum achievable volumes within the areas of limitations are accurately identified. A relief request from the code required examination.volumes is necessary. The final EPRI report will be on file at CPCO upon completion.
- PROPOSED ALTERNATIVE EXAMINATION All accessible weld volumes will be examined once per interval in lieu of the 100%
volumetric examination requirements.
