SUBJECT:
INTEGRATED INSPECTION REPORT 50-255/95011
Dear Mr. Palmisano:
This refers to the routine safety inspection conducted by M. E. Parker,
P. F. Prescott and others of this office from August 22 through October 11,
1995.
The inspection included a review of activities at your Palisades
Nuclear Generating Facility. At the conclusion of the inspection, the
findings were discussed with those members of your staff identified in the
enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
Based on the results of this inspection, we noted that certain of your
activities appeared tri be in violation of NRC requirements, as specified in
the enclosed Notice of Violation (Notice).
The violation is of concern
. because an alternative to the Code vibration acceptance criteria was
implemented prior to NRC approval, and in fact, the relief request was twice
denied because the limits ~stablished did not provide sufficient assurance
that pump testing would detect degradation prior to reaching the vibration
limits.
You are required to respond to this letter and should follow the instructions
specified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken* and any additional
actions you plan to prevent recurrence; After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
inspections, the NRC will determine whether further NRC enforcement actions is
necessary to ensure compliance with NRC regulatory requirements.
In addition, you continued to exhibit weakne~ses in foreign material control
and indications of poor plant material conditions.
Cracking of the generator
isophase bus connections, failure of a wire connection on the number two
governor valve linear variable differential transformer (LVDT), and failure of
the motor operated switchyard disconnects, led to two plant shutdowns and a
power reduction.
