INSPECTION REPORT NO 50-255/95007(DRP}

This refers to the routine safety inspection conducted by M. E. Parker,

D. G. Passehl, and others of this office, from April 13 through May 27, 1995.

The inspection included a review of activities at your Palisades Nuclear

Generating Facility. At the conclusion of the inspection, the findfngs were

discussed with those members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report. Within

these areas, the inspection consisted of a selective examination of procedures

and representative records, interviews with personnel, and observation of

acti~ities in progress.

The series of personnel errors that occurred during the initial phase of the

refueling outage are cause for concern. Although individually each was of

minimal safety significance, collectively they indicate a weakness in your

process for controlling plant activities, including communications, and

inattention on the part of plant personnel.

Of positive note is that your

operators identified each of these errors and you initiated corrective action.

Based on the results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation involved failure to take adequate

corrective action for sampling deficiencies in the radiological environmental

monitoring program. This violation is of concern because of repeated problems

in this area and past failures to correct the identified deficiencies.

You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.

During a review of our previous Inspection Report No. 50-255/95004, we noted

that an error was made in the numbering of unresolved item 255/95004-05(DRS}

on page 22, paragraph 5.1.1.2. Please change the number for this item from

255/95004-05(DRS) to 255/95004-07(DRS}.

We apologize for any inconvenience

this may have caused.

In accordance with 10 CFR 2.790 of the NRC's "Rules of Practice," a copy of

this letter, the enclosures, and your response to this letter will be placed

in the NRC Public Document Room.
