These events involved a through wall leak in a safe end to pipe weld in the pressurizer power operated relief valve (PORV) line and exceeding the Technical Specifications cooldown rate of 20 degrees per hour when the primary system cold leg temperature was less than 170 degrees. This letter provides supplemental commitments and clarifications related to the through wall leak in the pressurizer PORV line. Consumers Power Company will perform the following actions:

1.

Develop and submit a formal action plan related to the through wall leak inclu~ing those actions described in our September 29, 1993 letter and the actions listed below.

2.

Perform a comprehensive failure analysis and root cause evaluation of the pressurizer PORV line safe end crack.

The *nalysis will include evaluation of the implications of the root cause of the safe end crack to the other pressurizer penetrations safe ends and determine acceptability for continued operation.

3.

Perform inside diameter surface examination (where accessible) and volumetric NOE that is effective for the detection of IGSCC on similar pres~urizer Inconel 600 welds.

4.

Perform all code required repairs and associated post repair NOE to correct defects identified during the NOE.

5.

Re-evaluate the pipe stress associated with the modification to the piping and review the pressurizer spray line support.

6~ Document the results of the above actions in a report to the NRC.

This report is to include assurance why other similar austenitic (lnconel or stainless iteel) welds in the PCS, if any, do not require any additional examination or repair.

7. *Following submittal of the report, and prior to leaving cold shutdown, we request a meeting with NRC staff.
