27780 Blue Star Memorial Highway, Covert, Ml 49043 November 24, 199,2 Nuclear Regulatory Commission Document Control Desk Washington, DC 20555 GB Slade General Manager DOCKET 50-255 - LICENSE DPR PALISADES PLANT - CORRECTION TO CHANGE IN COMMITMENT - REPLY TO NOTICE OF VIOLATION (NOV) 9I-I7 NRC Inspection Report 9I-OI7, dated October I, I99I, documented the results of a special safety inspection and reported two apparent violations resulting from the inoperability of containment spray pump P-54C. This. condition was reported to-the NRC in Consumers Power Company's (CPCo) license~ event report (LER) 9I-OI6, dated August 22, I991.
The apparent violations were further -Oiscussed in an Enforcement Conference on October 9, I99I.. As a result of the inspection report and-the enforcement conference, a Notice of Violation (NOV 9I-I7) was issued on November I4, I99I, that identified two violations for (a) failure to establish adequate written procedures or instructions to ensure that the containment spray pumps, and similarly the high pressure safety injection pumps, were verified operable; and (b) operation of the reactor at power while containment spray pump P-54C was inoperable w,ithout complying with the applicable technical specification. _
By letter dated December 9, I99I, CPCo responded to NOV 9I-17, and in the response our "Corrective Action to Avoid Future Non-Compliance" included the following commitments:
I. Remote and local indication of closing coil power for safety related 2400/4I60V solenoid operated breakers for Buses IA, IB, IC and ID will be installed. This will provide the operator with positive indication as to-whether or not the closing coil circuit and the control power circuit is energized.
This action is scheduled for completion in the I992 refueling outage.

Remote and local indication of closing coil power for 2400/4I60V
- solenoid operated breakers for Buses IE, IF and IG will be installed.
. Breakers on these buses are non-safety related, therefore, delaying the installation of these indicators is not a safety significant issue.
- This action is scheduled for completion in the I993 refueling outage.
By letter dated November 6, I992, we informed the NRC that remote and local indication of closing coil power for safety related 2400/4I60V solenoid operated breakers had been installed on all the breakers in Buses IA, IB, IC and ID. That information was not completely accurate in that remote in.dication had not been installed for breakers I52-I04, Auxiliary Feedwater Pump P-BA (Bus Icrand.
breaker I52-209, Auxiliary Feedwater Pump P-BC (Bus ID); however, local i ndi cation was. added for these breakers.
The remote i ndi cation for these breakers was not added during the I992 refueling outage because sufficient time was not available during the outage to complete all the breaker modifications.
(Circuitry in twenty-nine circuit breakers was modified during the I992 refueling outage.)
Remote indication of closing coil power for these safety related*
2400/4I60V solenoid operated breakers I52-I04 and I52-209 will be added *during the I993 refueling outage.
-Finally, to reiterate the conclusion made in our Novembe~ 6, I992 letter, the 2400/4I60V solenoid operated breakers 6n Buses IF and IG will not be re-wir~d.
Re-wiring of the non-safety related, quality classed (i.e., Q-listed), 2400/4I60V
- solenoid operated brea_kers on -Bus IE, to provide remote and local indication, remains scheduled for completion during the I993 refueling outage.
