Dear Mr. Slade:
This refers to the inspection conducted by Mr. J. F. Schapker of
this office on February 25 through May 28, 1992.
The inspection
.included a review of authorized activities for your Palisades
facility.
The focus of this inspection was your inservice
inspection efforts and construction of the ventilated concrete
cask (VCC) temporary fuel s~orage facilities.
This report also
includes the special safety inspection of site activities
conducted by the Nuclear Regulatory Commission's Nondestructive
Examination (NDE) mobile team of the NRC's Region I office during
February 24 through March 6, 1992 (Enclosure 3).
At the
conclusion of the inspections, the findings were discussed with
those members of your staff identified in the enclosures to this
letter.
Areas examined during the inspection are identified in the
report.
Within these areas, the inspections consisted of
selective examination of.procedures and representative records,
observations of activities and interviews with personnel.
Weaknesses were identified both in the onsite construction of the
VCC temporary fuel storage facilities and in the implementation
of your ISI pr.ogram.c With regard to your VCC construction
project, oversight ot contractor activities was weak and examples
of work activities and QA/QC implementation were found during our
inspection which did.not meet applicable specifications and
standards.
These deficiencies did not appear to affect the
overall functionality of the VCC facilities because either they
were identified in time to take corrective action or were
analyzed, after the fact, to be of minimal significance.
However, we are concerned about your level of commitment to
ensure compliance with the applicable specifications and
standards.
We consider your actions to stop work on the project,
after being made aware of our concerns, to be a positive step.

Certain aspects of your inservice inspection activities appeared
to be in violation of NRC requirements, as specified inthe
enclosed Notice of Violation (Notice).
The violations are of .
concern because exami~ations required by the ASME Code, Section
XI and regulatory requirements were not performed or scheduled to
be performed.
Also, inspection rec~rds did not identify that
portions of a weld examination were not performed as required.
You are required to respond to this letter and should follow the
instructions specified in the enclosed Notice when preparing your
response.
In you_r response, *you should doc\\lment the specific
actions taken and any additional actions you plan to -prevent
recurrence.
In addition, the report addresses several unr~solved
items*.
We request that you also address each of these items
along with your reply to the cited violations identified in the
encl*osed Notice.
After reviewing youJ:'.' response to this Notice,
including your proposed corrective actions and the results of
future inspections, the NRC will determine whether further NRC
enforcement action is necessary to ensure compliance with NRC
- regulatory requirements.
In accordance with 10 CFR 2.790 of the Commission's regulations,
a copy of this letter, the enclosures, and your response to "this
letter will be placed in the NRC Public Document Room.
The- responses directed.by this letter and the accompanying Notice
are not subject to the clearance procedures of the Office of
Management and Budget as required by the Paperwork Reduction Act
of -1980, PL 96-51.1._
