Gentlemen:

This refers to the engineering team inspection conducted by Messrs. J. M.

Jacobson, J. A. Gavula and others of this office on September 19, 1990,

through April 18, 1991, of activities at the Palisades Nuclear Generating

Plant authorized by NRC Operating License No. DPR-20 and to the discussion of

our findings with Mr. D. P. Hoffman and others at the conclusion of the

inspection.

The enclosed copy of our inspection report identifies areas examined during

the inspection.

The focus of this inspection was directed toward activities

related to the Steam Generator Replacement Project. Areas inspected include

design engineering, field implementation, and testing. Within these areas,

the inspection consisted of a selective examination of procedures and

representative records, observations, and interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

of NRC requirements.

Multiple examples of apparent violations were identified

in the areas of design control, corrective actions and following procedures.

This inspection has again raised significant concerns in the area of design

control for the Palisades plant.

As I discussed with Mr. D. Hoffman, the NRC

has decided to conduct an additional inspection at Palisades in order to

better characterize these concerns.

As a result, no Notice of Violation is

being issued for the inspection findings in this report.You will be notified

at a l~ter data r~garding our decision on the findings of this inspection.

No

written response is required until you are so notified of our decision.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter and the enclosed inspection report will be placed in the NRC

Public Document Room .
