NOTICE OF VIOLATION Consumers Power Company Palisades Nuclear G~nerating Station Docket Nos. 50-255 License _Nos. DPR-20 As a result of the inspection conducted on March 4 through March 8, 1991, and in accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, (1991) (Enforcement Policy) the following violations were identified:

10 CFR 50, Appendix B, Criterion VI requires that measures shall be established to tontrol the issuance of d~cuments which prescribe all activities affecting quality.

These measures shall assure ttiat documents, including changes, are reviewed for adequacy and approved for release to authorized personnel and are distributed to and used at the l ocat.i on where the prescribed activity is performed.

Technical Specifications 6.8.1 requires that procedures be established, implemented and maintained.

Administrative.procedure 10.45, Vendor Manual -Control, requires that vendor manuals be identified, approved for use, revisions thereto and distr1bution controlled.

Contrary to the above, on March 7,.1991, numerous vendor manuals for portable health physics instrumentation, chemistry instrumentation and a component f6r an installed area radiation monitor were found lacking the required level of controls specified for such documents.

This is a Severity Level IV violation (Supplement I}.

Technical Specification 6.11 requires that procedures for personnel radiation protection shall be approved, maintained and adhered to for all operations involving radiation exposure.

Administrative procedure 7.01, Health Physics Portable Instrumentation Program, requires that all health physics instrumentation ~hall be calibrated, have operational checks performed once per day, shall be serialized, and shall have calibrat{on stickers placed upon them.

Contrary to the above, on March 7, 1991, a Ludlum Model 177 (count rate meter) that was not serialized, not calibrated and had not been operationally checked was found in use in the instrumentation and controls laborat6ry~

Contrary to the above, on March 4, 199i, a Radeco air sampler (# 7241)

. was found in room C-54 that had not been ope~ationally checked since

  • February 16, 1991.

This is a Severity Levei IV violation (Supplement IV).

Pursuant to the prov1s1ons of 10 CFR 2.201, you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for*each violation: 11) the corrective.ste~s that have been taken and the results achieved; (2) _the corrective steps that will be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
