This refers to the routine safety inspection conducted by Messrs. I. T. Yin
and D. S. Butler of this office on March 13-15, April 2-6, and May 9, 1990,
of activities at the Palisades Nuclear Generating Plant authorized by NRC
Operating License No. DPR-20, and to the discussion of our findings with
Mr. D. J. VandeWalle at the conclusion of the inspection.
The enclosed copy of our inspection report documents our review of activities
associated with your Configuration Control Project (CCP) evaluations for the
Component Cooling Water, Class 1 battery, 125 VDC power supply, and 4 kv AC
power supply systems.
The inspection focused on those portions of the CCP
dealing with Design Basis Documentation (DBD) preparation; Safety System
Design Confirmation (SSDC) performance, which is a detailed vertical slice
review of the system design and performance; and corrective action
implementation for the discrepancies identified by these efforts. Within
these areas, the inspection consisted of a selective examination of procedures
and representative records, observations, and interviews with personnel.
We recognize the extensive resource commitment required in this effort, which
has resulted in several significant findings.
Although there were many
discrepancies identified that had not been characterized as to significance,
we note that you have increased_ the amount of resources applied to the
performance of these characterizations.
The results of this inspection indicate that your CCP efforts in the electrical
area have been well conducted and documented many significant safety issues.
Corrective actions and schedules were appropriate.
There were several examples
where discrepancies between the fuse sizes specified on the drawings and those
installed in the plant resulted in changes to the drawings without sufficient
engineering evaluation to determine whether the drawing or current installation
was acceptable.
We understand that you are developing a program to ensure
fuse usage at Palisades is appropriate.
The CCP efforts in the mechanical area have been extensively audited, and
based on those audit results, the necessity for significant improvements in
the DBD process have been identified.
As a result of these improvements, the
original commitment to complete all CCP efforts by 1991 will not be met, and
your current schedule will extend completion into 1993.
This schedule appears
to be reasonable based upon the scope of remaining work.

inspection.
