Jackson, MI 49201 Gentlemen: MAY 2 2 1987 This refers to the routine safety inspection conducted by Mr. E. R. Swanson and Miss C. D. Anderson of this office during the period of April 7 through May 4, 1987, of activities at the Palisades Nuclear Generating Plant authorized by NRC Provi-sional Operating License No. DPR-20 and to the discussion of our findings with Mr. D. P. Hoffman and others of your staff at the conclusion of the inspection. - The enclosed copy of our inspection report identifies areas examined during the inspection. Within these areas, the inspection consisted of a selective
- examination of procedures and representative records, observations, and
interviews with personnel. During this inspection, certain of your activities appeared to be in violation of NRC requirements, as specified in the enclosed Notice. A written response is required. The enclosed report details several operations related events (Paragraph 3.d, e. and h.) apparently caused by lack of attention to detail, but also related to inexperience on the part of the operati-0ns crew involved. Please include with your response to the violations a discussion of your assessment of root cause(s) and corrective actions you have taken and plan to take to prevent recurrence. In accordance with 10 CFR 2.790 of the Commission 1s regulations, a copy of this letter and enclosures, and your response to this letter will be placed in the NRC Public Document Room.
