NOTICE OF VIOLATION Consumers Power Company Docket No. 50-255 As a result of the inspection conducted during the period May 28 through July 22, 1986, and in accordance with the "General Policy and Procedures for NRC Enforcement Actions, 11 10 CFR Part 2, Appendix C, (1985), the following violation was identified:

10 CFR 50, Appendix B, Criterion. II states in part:

"The Quality Assurance Program shall provide control over activities affecting the quality of identified structures systems and components, to an extent consistent with their importance of safety.

11 Consumers Power Quality Assurance Program Chapter 2.2.3.6 states:

11The Quality Assurance Program applies to activities affecting the quality of safety-related structures, systems, components, and related consumables during plant operation, maintenance, testing, and all modifications.

These activities having a direct impact on Q-listed items shall be procedurallY. controlled.

11 Palisades Administrative Procedure 4.03 Revision l, 1Equipment Control, 11 Paragraph 9.0, 11Jumper, Links, and Bypasses 11 identifies the controls required for installation of 11temporary lines.

11 Specifically, Paragraph 9.12.g requires 11 Non-routine operating, testing, troubleshooting, and maintenance functions involving bY.passing shall be documented on a Jumper, Link, and Bypass Control Log. 1 This log requires certain approvals and verifications.

Contrary to the above, appropriate controls were not implemented during the installation of a temporary. gauge on the Q-listed Emergency. Diesel Generator 1-1 lubricating oil system on May 26, 1985.

This gauge subsequently became a source of an oil leak and led to the diesel being declared inoperable on June 22, 1985.

This is a Severity Level IV violation (Supplement I).

Technical Specification 6.8.1.a requires that written procedures shall be established, implemented, and maintained for applicable procedures in Appendix 11A 11 of Regulatory Guide 1.33 endorsed by CPC-2A which includes procedures governing log entries.

Administrative Procedure 4.01, 11Shift Operations, 11 Paragraph 5.7.2.b.4 requires that the Shift Supervisor 1s Log contain notifications and reportable occurrences.

Contrary to the above, on June 17, 1986, the Shift Supervisor 1s Log did not contain an entry for the diesel generators (DG) actuation which is a reportable occurrence or for the 10 CFR 50.72 notification of same.

Also no entry was made for the 10 CFR 50.72 notification concerning the DGs actuation of June 10, 1986.

This is a Severity Level V violation (Supplement 1).

Date With respect to Item 1, the inspection showed that action had been taken to correct the identified violation and to prevent recurrence.

Consequentiy, no reply to the violation is required and we have no further questions regarding this matter.

With respect to Item 2, pursuant to the provisions of 10 CFR 2.201, you are required to submit to this office within 30 days of the date of this Notice a written statement or explanation in reply, including for each violation:

(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further violations; and (3) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown.
