This refers to a special onsite investigation review conducted by the NRG
Augmented Investigation Team on May 22-25, 1986 relating to concerns over
multiple equipment failures at the Palisades facility, as demonstrated by
the May 19 reactor trip and associated equipment failures, the potential
for serious challenges to safety systems such failures pose, and the burden
these failures place on your operating staff in order to maintain the plant
in a safe operating condition. This also refers to discussions of findings
with yourself, Mr. J. F. Firlit, and others of your staff during the course
of and at the conclusion of the onsite AIT review, and a meeting with the
NRG staff at the Region III office on June 6, 1986.
The enclosed copy of the Augmented Inspection Team report identifies areas
examined during this review.
Within these areas, the investigation consisted
of a review of representative records, observations, and interviews with
personnel.
In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure
will be placed in the NRG Public Document Room.
We will gladly discuss any questions you have concerning this inspection.
