RESPONSE TO INSPECTION REPORT 85-011 David J VandeWalle Director of Nuclear Licensing One item of noncompliance was identified in Inspection Report 50-255/85-011 dated May 31, 1985.

The following is our response to this item:

Noncompliance 50-255/85-011-01 Technical Specification 6.8.1.a states that written procedures shall be established, implemented and maintained covering the applicable procedures recommended in Appendix "A" of Regulatory Guide 1. 33.

Section 7 of Appendix "A" of Regulatory Guide 1.33 requires establishment of procedures for control of radioactivity (for limiting materials released to the environment).

a)

Procedure No. HP 6.4, "Radioactive Liquid Calculation and Release Authorization" in Section 5.4.h.2 requires that the MPC for I-131 be substituted in the release rate equation in place of the MPC value of 2E-05 when I-131 activity is present in the batch.

Contrary to the above, for liquid release batch Nos. 85-004-R, 85-006-R, and 85-007-R, released in early 1985, and for which I-131 activity was identified as present in each batch, the MPC for I-131 was not substituted into the equation as required by the procedure.

b)

Form HP 6.4-la, "Release Order", of procedure No. HP 6.4 provides for documentation of required release rate in gpm as approved and signed for by the Shift Supervisor prior to the release as well as subsequent documentation by Auxiliary Operators of the release rate indication in gpm and the actual release rate in gpm based on volume of release and duration of the release which are to be within the approved release rate.

Contrary to the above, for Release Batch No. 85-017-R, the Shift Supervisor authorized a release rate of less than 25 gpm whereas the OC0685-0012B-NL02

indicated release rate was set at 30 gpm and the actual calculated release rate was 32 gpm.

Response (a):

Corrective Actions Taken and Results Achieved A review of the release rate calculations determined that the concentrations in each release did not exceed the MPC value.

Corrective Actions To Be Taken To Avoid Further Noncompliance Health Physics Procedure 6.4, "Radioactive Liquid Calculation and Release Authorization", will be revised to require the Iodine-131 MPC (3E-07) to be the normal value utilized in calculations.

The procedure will note that the Cesium-137 MPC (2E-05) may be substituted when Iodine-131 is not present.

Using this approach, a more conservative release rate will be calculated in those cases where a substitution is not performed.

Date When Full Compliance Will Be Achieved Full compliance will be achieved by July 30, 1985

Corrective Actions Taken And Results Achieved The error was reviewed and determined to represent an isolated occurrence.

The release did not result in a violation of any limits.

Operations personnel were notified of the occurrence and the potential for a similar error to result in a violation of Technical Specifications.

Corrective Action To Be Taken To Avoid Further Noncompliance All corrective actions are complete.
