20555 David J VandeWalle Director of Nuclear Licensing Enclosed is additional information related to thePalisade.s Steam Generator auxiliary feedwater sparger, as promised in my letter dated June 26, 1984.
The following corrective action docillnents describing the disposition of various failures of the old sparger and liner are attached:

E-PAL-84-016, "Broken Auxiliary Feed Ring Connection" Note that the proposed corrective actions identified on page 3 of the document were not performed.
These actions involved a new thermal sleeve and spiash shield designed by Combustion Engineering, as well as procedural controls to minimize potential adverse operating conditions (low flow rates with the sparger uncovered}.
The Westinghouse design supercededthese corrective actions and addressed all design deficiencies identified in the previous design.
