Description of Incident:

The previous exceedances of the daily maximum limits for oil and grease on March 10, 11, 12 and 15 (as reported to staff on March 29, 1982) resulted in the monthly average for March being 20.2 mg/l.

Apparent Cause:

Fuel oil loss from heating boiler 'reached building drain system as previously reported to staff ~n March 29, 1982.

The source of the fuel oil leakage was repaired.

The incident numbers were incorrect in the previously submitted reports.

A change in plant operating conditions requiring the pH measurements to be made was not recognized.

The importance of required NPDES monitoring was stressed to each technician and data are now being tabulated in a manner to facilitate more timely review.

Oil and grease are limited to 20 mg/l daily maximum concentration.

Description of Incident:

On 3/10/82, a large quantity of fuel oil reached the tuFbine sump resulting in a discharge of 50.2 mg/l oil and grease.

As a result of this upset, the following values were also recorded:

3/11/82, 188.1 mg/l 3/12/82, 63.8 mg/l 3/15/82, 55.5 mg/l

Apparent Cause:

Fuel loss from leaking components on the Service Building heating boiler reached the building drain system.

If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Maintenance orders were immediately issued to repair the leaking components on the Service Building heating boiler.

On 3/16/82 and 3/19/82, the oil and grease values measured 16.4 mg/l and 2.2 mg/l, respectively.

Description of Incident:

On 1/11/82, the oil and grease was reported as 1.3 mg/l; on 1/12/82, it increased to 25.5 mg/l; and by 1/14/82, it was back to 9.0 mg/1.

The duration of the incident was estimated at eight hours.

4.

Apparent Cause:

It is believed that residual oil from a previous event was still present in one of the drain lines and was carried through the system.

5.

If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action ~ The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

The cause of this, and th~ previous two incidents, was a plugged vapor drain line on an oil system which is currently under repair to prevent reoccurrence.

Description of Incident:

On 1/7/82 the process was sampled and analyzed at 0.4 mg/l.

The 1/8/82 sample indicated 25.4 mg/l.

4.

Apparent Cause:

The continued cleanup of the previous (1/5/82) problem probably caused additional oil to reach the floor drain system.

5.

If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Additional efforts are under way to eliminate oil sources from reaching floor drains.

Description of Incident:

During the week preceding this incident, attempts were being made to bring the turbine generator on line and problems were experienced with the generator bearing oil seals.

The problems resulted in losses of significant quantities of lubricating oils to the floor of the turbine building.

The operations group was very attentive to the leakage and they did as much as possible to control the spread of the oil to the floor drains.

The preconditioner and emulsion breaker filters were changed on the 1st of January due to an identi-fied breakthrough.

Ori the 5th of January, the filters were again changed out after routine inspection of the filter effluent indicated the filters had failed.

The effluent was sampled and shown to be less than 20 mg/l on January 4.

The sample on January 5 indicated 43 mg/l and the sample on the 6th measured 5.2 mg/l.

Apparent Cause:

Overtaxing of the system wit~ large losses of oil to the floor from the generator bearing oil seals.

Monthly average limit of 30 mg/l for total suspended solids (TSS).

3.

Description of Incident:

On October 14, 1981 the daily maximum limit for TSS was exceeded (120 mg/l TSS) by an amount sufficiently great enough to cause the monthly average TSS value to also be exceeded (31.86 mg/l TSS).

4.

Apparent Cause:

The reason for exceedance of the monthly average TSS limit was exceedance of the daily maximum TSS limit on 10/14/81, which was explained in our previous NPDES Notification Report of 10/23/81 (PAL-81-10-AD60).

5. If not immediately corrected, the anticipated time the condition is expected to continue:

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Corrective action to address the exceedance of the daily maximum TSS limit was previously discussed in the NPDES Notification Report of 10/23/81 (PAL~81 AD60).

The average of all releases *other than the 10/14/81 release was less than 10 mg/l TSS.

MWRC NOTICE OF NONCOMPLIANCE (WRC No NC-11-81-07-4096)

On November 9, 1981, MWRC issued the subject Notice of Noncompliance for the Palisades Plant regarding apparent exceedances of permit limits for outfalls OOD (laundry wastes prior to dilution) and OOG (turbine sump oil separator wastes).

In response to your request for written explanation, the following is provided.

As acknowledged in the MWRC Notice, our previous NPDES notification reports of October 13 and 20, 1981, addressed the September average and September 10, 1981, exceedances for outfall OOD and the September 29, 1981, exceedance for outfall OOG.

The NPDES notification report for the September 3, 1981, exceedance for outfall OOG is attached.

In regard to the September 29, 1981, sample value for out fall OOG, we question the inclusion of this sample value as an exceedance of permit limits.

The daily average limit for oil and grease for outfall OOG is 15 mg/l.

  • The daily average value for September (calculated at the end of the month) was ll. 8 mg/1.

It is my understanding that a single value during the month greater than 15 mg/l but less than the daily maximum limit of 20 mg/l does not constitute an.exceedance of thP. daily average value since the daily average is calculated (the average of c.11 readings during the month) at the end of the month.

We feel the above adequately explains this incident.

If my understanding of this matter is incorrect, please let me know.

The use of phosphate-based soaps for laundry and respiratory equipment cleaning resulted in the phosphorus content of the batch released exceeding 2 mg/l.

An unanticipated high use occurred during this period.

This was a single release and was terminated the same. day.

The oil and grease concentration in the turbine building drain tank discharge was reported as.31 mg/l on 9/28/81.

Apparent.Cause:

On 9/28/81, steam generator water was released to the turbine building sump due to a relief valve being removed for repair.

It is believed that the water treatment chemicals in this water caused* an imbalance in the system allowing oil to be carried through in an emulsified form rather than separating out.

. A sample collected from Tank T-41 at 1620 hours0.0188 days <br />0.45 hours <br />0.00268 weeks <br />6.1641e-4 months <br /> on 8-10-81 was 35.0 mg/l.

Apparent Cause:

The emulsion breaker portion of the oil removal system failed as a result of a plant power upset at 1250 hours0.0145 days <br />0.347 hours <br />0.00207 weeks <br />4.75625e-4 months <br /> on 8-10-81.

As a result of a system pressure surge during the power upset, oil was pushed through the treatment system.

If not immediately corrected, the anticipated time the condition is expect7d to continue:

A follow-up sample collected at 0840 on 8-11-81 contained 2.4.mg/l oil and grease.

Corrective Action - The following corrective action was taken or will be taken to reduce, eliminate and prevent recurrence:

Upon observing that system overpressurization had occurred, Tank T-41 dis-charge was closed, Tank T-41 and the turbine building sump were inspected and the emulsion breaker filters were immediately replaced.

Additional Comments:

It was determined from an evaluation of the occurrence that the likelihood of this situation recurring is remote.

A high DP alarm was received by the control room at 1015 on 7/17/81.

At 1020, the system was secured and the filter elements replaced; by 1120, the system was back in service.

At 1430, the passive system in T-41 (oil-sorbe-nt pillows and passive skimmers) were charged and drained.

Apparent Cause:

A portion of the turbine sump oil separator wastewater system was taken out of service for maintenance and system evaluation for repair.

MDNR Staff was notified *of the partial* system bypass prior to the bypass.
