Appendix A NOTICE OF VIOLATION Consumers Power Company Docket No. 50-255 As a result of the inspection conducted on October 4-30, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
- 1.
Technical Specification 6.8.1.a requires written procedures be im-plemented covering activities listed in Appendix A of Regulatory Guide 1.33 (November 1972) which includes, at F.23, procedures for combating plant fire emergencies.
Plant Fire Protection Implementing Procedures, Section 7, Paragraph 5.5.3 requires a firewatch who shall have a suitable fire extinguisher readily available at hand when welding, cutting, or grinding.
Contrary to the above, on October 14, 1981, welding was conducted at the containment 649-foot level fabrication area when no fire extin-guisher was available at hand.
This is a Severity Level V violation (Supplement IE).

Technical Specification 6.8.1.b requires written procedures be estab-lished *and implemented covering refueling operations.

Procedure SOP-28, "Fuel Handling Systems" requires at Section 3.1 that when loaded with spent fuel, the two-speed elevator gearbox shall be mechanically restricted to prevent gear changes.
Contrary to the above, on October 5, 1981, the licensee changed speeds on the gearbox with a spent fuel bundle in the elevator.
- b.
Procedure GOP-11, "Refueling Operations" requires, at Step 3.2, completion of checklist G CL 11.2 during each shift of fuel
. handling.
Contrary to the above, Checklist G CL 11. 2 was not completed during shifts of fuel handling on September 25-27, 29, 30 and October 1-6, 1981, in that Items 11, 12, and 13 were variously omitted.
This is a Severity Level V violation (Supplement IE).*
Technical Specification 6.8.1.a requires written procedures be estab-lished covering activities listed in Appendix A of Regulatory Guide _l. 33 (November 1972) which includes, at G.1.a, liquid radioactive waste collection.
Technical Specification 6.8.2 requires review and approval of these procedures by the Plant Review Committee and the plant manager, respectively, prior to implementation.

J Appendix A Contrary to the above, the licensee performed liquid radioactive waste collection from the containment sump on October 3, 1981, via a test con-nection between the containment isolation valves without benefit of a procedure.
This is a Severity Level VI violation (Supplement IF).
With respect to Items 1 and 2a, the inspection showed that action had been taken to correct the identified items of noncompliance and to prevent recurrence.
Consequently, no reply to these items of noncompliance is required and we have no further questions regarding this matter.
With respect to Items 2b and 3, pursuant to the provisions of 10 CFR 2.201 you are required to submit to this office within thirty days of the date of this Notice a written statement or explanation in reply, including for each item of noncompliance:
(1) corrective action taken and the results achieved; (2) corrective action to be taken to avoid further noncompliance; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be. submitted under oath or affirmation.
Con-sideration may be given to extending your response time for good cause shown.
