Gentlemen:
This refers to a conversation between you and your staff and myself on January 9, 1981 regarding your actions to prevent reoccurrences of events similar to that which occurred on January 6, 1981.
Based on this con-versation we understand that you will immediately put into effect the following:
- 1.
Conduct an audit of plant operations daily by a corporate management representative.
- 2.
Establish a committee comprised of a member of corporate management (in addition to the corporate representative referenced in item 1 above), a Senior Reactor Operator, and another qualified engineer to review, prior to use, all safety-related surveillance and maintenance procedures and other maintenance procedures which cover work to be conducted in vital areas to assure that:

- f.
Each procedure is specifically identified as being safety related, or as having the potential to affect safety-related equipment; Authorization to perform work is required from plant management; Special notification of work performed is made ~'the Shift Supervisor;

System conditions to perform work are defined; Minimum personnel skill-level is defined; and Return-to-normal verification requirements are specified.
Credit can be taken for the review performed in response to the NRC's November 9, 1979 Order.

Reinstruct all personnel who perform safety related work or other work in vital areas on the importance of strict adherence to procedures, and the necessity for performance of all assigned duties in a disciplined and professional manner.
- 4.
Verify, by a second qualified individual iDDDediately upon completion, all activities involving the manipulation of safety related circuits or systems.
Qualified individuals will be designated by the Plant Manager for the specific tasks.
- 5.
Review the specific circuitry involved in the January 6, 1981 event to determine if control room indications are required to show when an abnormal line-up exists. Circuit modifications, found necessary, will be performed on a schedule to be agreed to by Region III.
Please inform us immediately if your understanding of these items is different from that stated above.
This is a matter of serious concern to the NRC.
These items are intended to address only the short term actions, and further long term requirements are under review.
