for *the diesel *generators at their facil 1ties. Thi.s request for informa..:

tion was prompted by reports *from_ liCensees, Le;, LE.Rs, on incidents where diesel generators. faHed to respond *to automatic start' signals because

  • coritrol switches or lockout arid shutdown relays.were left in the shutdown condition by operators unaware of their status~*. The principle reasons for
  • this lack of awareness were:

(1) the sharing of annunciator stattons for

  • both disabling and non-disabling alaY'.fTI conditions;. (2) wordfog on annunciator

.windows for disabling conditions whicti did not specifically say a diesel generator eme~ency_start was b~.ocked; and (3) disabling.ccmditions which were not annunciated i.n.the control room.

Your letters dated May i6, 1977 and July 12, 1977, responded.to our generic letter and described the diesel generator.control circuitry and annunciator system at the Palisades Plant. Quring OU}'.'. original review of your.response, several areas of. concern were identified. These are:

Thus, *we consider items 3,.4. and 5.above to be resolved.

Items 1 and 2,* however;, require corrective actions as de"scri bed be 1 ow.

There are currently six disabling conditions which are aianned at the.

same annunciator point (window labeled "DIESEL GENERATOR TROUBLE") as alarms that only *indicate a warning of abnonnal. but not...cUsabl fog,.
