~!e have reviei:red your submittal dated July 6, 1978, pertaining to the PWR Moderator Di)ution analysis and find that your analysis may be incomplete.
We had requested licensees to revi_ew the potential for all types -of boron dilution incidents.
From the information subnlitted-it is not clear that this was done* for -Palisades.
Your submittal only addresses the potential for the boron dilution incident that could occur by inadvertent draining of the spray additive tank to the reactor coolant sy~tem as occurred at Crystal River.
To complete our review we need to know if you have evaluated the-,
potential for other boron dilution accidents not analyzed in the FSAR and the consequences of _any boron djlution accidents so identified.
Your submittal identifies a potential event that, with a single failure, would result in the transfer of unborated water from the fodine Removal System to the Primary Cool ant System such that the reactor would return to criticality.
To reduce the likelihood of this event, you Ii.ave initiated adrninistrativ'e controls to isolate the Iodine Removal System from the. Shutdo\\'m -Cooling System during pl ant outages.
With respect to these contrO"ls we request that you submit:
(1) a detailed description of the administrative controls to preve~t the event; (2) the time available for operator action to prevent criticality following the-low level alarm (3900 gallons) from the Sodium Hydroxide Storage Tank (T-103_);
(3) the error band in the determination of-the critical boron concentration; and (4) an evaluation as to whether criticality would,be achieved prior to reaching the low level a-larm setpoint in the third situation postulated in your July 6 letter.
