Appendix A NOTICE OF VIOLATION Consumers Power Company License No. DPR-20 Based on the inspection conducted on October 24-25, 1978, it appears that certain of your activities were not in compliance with NRC requirements, as noted below.
This item is a deficiency.
Palisades technical specificaions Section 6.10.le requires records of training and qualification for members of plant staff to be retained for five years. Administrative procedure 13 delineates specific training requirements and records.
Contrary to the above, records of training for several employees did not exist in the following areas:
- 1.
New employee training orientation within two days of employment as required by Administrative Procedure 13.1, Section 5.2a.
- 2.
Respiratory protection indoctrination training for new employees as required by Administrative Procedure 13.1, Section 5.2c.
- 3.
Annual job related quality assurance instruction and biennial training in use of forklift, crane safety, scaffolding/rigging as required by Administrative Procedure 13.3.3, attachment 13.3.3.
- 4.
Quarterly job related Industrial/Fire Saf.ety Training as required by Administrative Procedure 13.3.6, attachment 13.3.6.
- 5.
Semiannual site emergency drill participation as required by Administrative Procedure 13.4, attachment 13.4B.
