Appendix A NOTICE OF VIOLATION Consumers Power Company Docket No. 50-255 Based on the inspection on September 25-29, 1978, it appears that a certain activity was in noncompliance with NRC requirements, as noted below.
This item is an infraction.
The Palisades Technical Specification 6.8.1 requires that written proce-dures shall be established and implemented as recommended in Appendix A to Regulatory Guide 1.33 (November 1972) which includes procedures for the control of maintenance activities. Administrative Procedure 1.5, Section 5. L 1.1 requires that prior to declaring the equipment "operable",
a review of the maintenance documentation must be performed **** and that suitable documentary evidence exists to substantiate acceptable comple-tion and the maintenance.
Contrary to the above, the control rod drive mechanisms were declared "operable" following completion of the maintenance activity during the last refueling outage (January 6 through April 1, 1978) without comple-tion of the administrative review and disposition of discrepancies.
