SUMMARY

OF OPERATING EXPERIENCE FOR SEPTEMBER 1978 9-1-78 9-1-78 9-8-78 9-12-78 9-13-78 9-16-78 9-17-78 9-22-78 At the beginning of the month, the plant was shutdo~m in continuation of an outage which began on August 28, 1978.

Amendment 42 to the Palisades Operating License was issued by the NRC.

This amendment related primarily to fire protection equipment and plant modifications associated with fire protection.

Amendment 43 to the Palisades Technical Specifications was issued by the NRC.

This amendment changed the specifications relating to axial power distribution limits.

The plant was returned to power operation, ending an outage of 267 hours0.00309 days <br />0.0742 hours <br />4.414683e-4 weeks <br />1.015935e-4 months <br /> duration.

During the outage, seven CRDM Seals were replaced, and the seals for primary coolant pumps B and D were replaced.

Because of a malfunction to the newly installed seal in the B primary coolant pump, the seal required a second replacement during the outage.

In addition to the above, a weld repair was performed to correct leakage in the Chemical and Volume Control System (CVCS) letdown line.

The plant was shutdown to permit repairs to the seal of CRDM No. 39.

The plant was returned to power operation, ending an outage of 78.9 hours1.041667e-4 days <br />0.0025 hours <br />1.488095e-5 weeks <br />3.4245e-6 months <br /> duration.

In addition to CRDM No. 39, five other CRDM seals were replaced with rebuilt seals.

Because of the inability to withdraw CRDM No. 39, the rod's control fuses were removed and the rod was declared inoperable.

The drive mechanism failure mode indicated that the failure had occurred in the drive package, and that the rod had retained its tripping capability. Refer to LER 78-031.

The plant was shutdown to permit repairs to CRDM seals.

The plant was returned to operation on September 26, after a 94.6 hour6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> outage.

Six CRDM seals were replaced.

The drive package for CRDM 39 was replaced, and the rod was restored to an operable status (LER 78-031).
