This routine inspection of the Susquehanna
Steam Electric Station covered
plant operations,
surveillance,
maintenance,
open
items followup, Unit 2 First
Refueling Outage Activities, allegation followup, and licensee
events.
The
inspection
focused
on Unit 2 refueling outage activities and followup of
events.
No violations were identified.
During this report period,
three
events involving waterhammers
occurred
in
plant systems;
two of which resulted in hanger
damage.
Management attention
should
be focused
on determining
the root cause of these
events
on
establishing
and implementing effective measures
to prevent their recurrence.
Our review of these
events
found that all occurred during non-routine
evolutions not specifically covered
by detailed plant procedures.
Additional
operator training to increase
the sensitivity to waterhammer
events
and
methods of prevention
appears
warranted.
