NRC INTEGRATED INSPECTION REPORT 50-259/96-13,

50-260/96-13

AND 50-296/96-13

NOTICE OF VIOLATION

Dear Mr. Kingsley:

On January 4, 1997. the

NRC completed

an inspection at your Browns Ferry

facility.

The enclosed

report presents

the results of that inspection.

During the six-week period covered by this inspection,

your conduct of

activities at the Browns Ferry facility was characterized

by safety-focused

lant operations

and timely and accurate

engineer ing assessments.

The plant

as experienced

recurring failures of vital

DC equipment in the last several

months,

including vital inverters

and battery char gers.

Efforts to mitigate

failures appear

adequate

-however, root causes

have not been determined.

Two

violations of NRC requirements

were identified.

The .first violation addresses

inadequate

work control implementation of activities involving placement of a

cover over the Unit 2 spent fuel pool.

The second violation addresses

two

examples of inadequate

procedures

which resulted in an uncontrolled high

radiation area

and the subsequent

unplanned

exposure to plant personnel.

These violations are cited in the enclosed

Notice of'iolation (Notice),

and

circumstances

surrounding the violations are described in detail in the

enclosed report.

and should follow the instructions specified in the enclosed

Notice when

preparing your response..

The 'NRC wH1 use your response,

in part, to

determine whether further

enforcement action is necessary to ensure

compliance

with regulatory .requirements..
