DPR-68 During an NRC inspection conducted from October 13 through November 23, 1996.
one violation of NRC requirements was identified.
In accordance with the "General Statement of Policy and Procedure for NRC Enforcement Actions."
NUREG-1600, the violation is listed below:
Technical Specification 6.8.1.l.a requires that written procedures shall be established, implemented, and maintained covering the applicable procedures recommended in Appendix A of Regulatory Guide 1.33.
Revision 2, February 1978.
Paragraph 4 of Appendix A of Regulatory Guide 1.33 recommends procedures for operation of safety-related BWR systems.
Procedure 0-0I-30F, Common and DG Building Ventilation. Revision 15, provides prestartup/standby readiness requirements and procedural steps for operation of the diesel generator and intake building ventilation.
Section 4.1.2.4 requires that the system lineup checklists are current using Attachment 2A, Common Building Ventilation Panel Lineup Checklist, Unit 3.
Attachment 2A states that the required position of the diesel auxiliary board room 3EA and 3EB exhaust fan handswitches 3-HS-030-0243 and 3-HS-030-244 is ON.
Note (1) to this requirement states that unless maintenance is being performed, these fans should be kept in service to revent excessive temperatures when the diesel generator 480V auxiliary oards are energized.
Procedure O-GOI-300-1, Attachment 15.18, dated November 15, 1996.
provides Operations with routine checklists 'for position of Unit 3 Control Bay heating and ventilation handswitches and equipment position.
Corrective Actions, in par t, for LER 260/89022, Rl, were to pr epare appropriate plant instructions addressing the operation of the exhaust fans for the diesel generator auxiliary board room by February 2, 1990.
Contrary to the above, Procedure'-OI-30F was not followed, in that on November 19,
- 1996, the diesel generator auxiliary board room 3EA and 3EB exhaust fan handswitches 3-HS-030-0243 and 3-HS-030-0244 were found by the inspectors to be in the OFF position, the fans were not running, and no maintenance was being performed.

- 1996, procedure 0-GOI-300-1 was found inadequate in that it did not require verification of proper. operation of the diesel auxiliary board room 3EA and 3EB exhaust fans.
This is a Severity Level IV violation (Supplement I).

Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington, D.C.
20555 with a copy to the Regional Administrator, Region II, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter transmitting this Notice of Violation (Notice).
This reply should be clearly marked "Reply to a Notice of Violation" and should include for each violation: (1) the reason for the violation, or, if contested, the basis for disputing the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken. to avoid further violations, and (4) the date when full compliance will be achieved.
Your response may reference or include previous docketed correspondence, if the correspondence adequately addresses the required response.
If an adequate reply is not received within the time specified in this Notice, an order or a Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken.
Where good cause is shown, consideration will be given to extending the response time.
Because your response will be placed in the NRC Public Document Room (PDR), to the extent possible, it should not include any personal privacy, proprietary, or safeguards information so that it can be placed in the PDR without redaction.
