,Mr. Oliver D. Kingsley, Jr.
President,
TVA Nuclear
and
Chief Nuclear Officer

SUBJECT:
NRC
INSPECTION
REPORT
NOS. 50-259/95-51,
50-260/95-51,
50-296/95-51,
Dear Mr. Kingsley:
This refers to the inspection
conducted
on August
13 to September
16,
1995 at
the Browns Ferry facility.
The purpose of the inspection
was to determine
whether activities authorized
by the license
were conducted 'safely in
accordance
with NRC requirements.
At the conclusion of the inspection,
the
findings were discussed
with those
members of your staff identified in the
enclosed
report.
Areas
examined during the inspection
are identified in the report.
Within
these
areas,
the inspection consisted of selective
examinations of procedures
and representative
records,
interviews with personnel,
and observation of
activities in progress.
Based
on the results of this inspection,
the
NRC has determined that
a
violation of NRC requirements
occurred.
This violation is cited in the
enclosed
Notice 'of Violation and the circumstances
surrounding it are
described
in detail in the subject inspection report.
We are concerned that
despite
several
significant foreign material
problems at Browns Ferry, foreign
material
exclusion practices
were not adequately
strengthened.
The violation described
in the enclosed
Notice of Violation is similar to a
violation contained
in the Notice sent to you by our letter dated
December
7,
1994.
Recurring violations are of particular concern
because
the
NRC expects
licensees
to learn from their past failures
and to take effective corrective
actions.
The Enforcement Policy states
that the severity level of a
repetitive Severity Level
IV violation may be increased
to reflect the
added regulatory significance of the violation.
In this case,
we have decided
not to hold
an enforcement
conference
nor to increase
the
severity level of the violation because
the proposed corrective actions for
the previous violation had not yet been fully implemented.
In your response
to the enclosed
Notice, you should
document
the specific actions
taken
and
any
additional
actions
you plan to prevent recurrence.
We will .review your
response,
including your proposed corrective actions,
and the results of
t
future inspections
to determine
whether further
NRC enforcement
action is
necessary
to ensure
compliance with NRC regulatory requirements.
