This letter refers to the special
maintenance
team inspection
led by
Mr. E.
H. Gray of this office on November 28,
1988 to December
9,
1988, princi-
pally at the Nine Mile Point Power Station, Unit 1, Scriba,
Mr . Gray
discussed
the findings of the inspection with members of your staff during
the conclusion of the inspection.
\\
The inspection
team concluded that Niagara
Mohawk Power Corporation
implements
an effective maintenance
program.
Overall, the material condition of the
plant, the experience
and knowledge of onsite personnel,
the onsite controls
on
the performance of maintenance,
the inte'raction
between
maintenance
and operations,
and the documentation
of maintenance activities were considered
good.
However,
corporate
management
needs
to strengthen
the maintenance
program,
through:
( 1)
enhanced
documented'direction
and goal setting;
(2) additional technical staff
to address
a growing maintenance
and outstanding
issues
backlog; (3) increased
involvement
and support
from corporate
engineering;
and (4) improved oversight
of performance,
effectiveness
and timeliness of corrective actions.
We note
that many of these findings are consistent with the conclusions
stated
in
the Niagara
Mohawk, Nine Mile Point Unit
1 Restart Action Plan,
published in
December,
1988.
Corrective actions outlined in the plan, if timely and thorough,
should resolve
many of these
concerns.
The details of specific problem areas
or weaknesses
that warrant your further
consideration
are discussed
in the report
and are outlined in Appendix 3.
Of
these
items,
prompt resolution is warranted for the potential of paint flakes
and rust to clog containment
or core
spray nozzles;
the testing of air operated
valves to demonstrate
achievement
of fail safe position
on loss of air; the
control of timeliness
and quality of resolution to problem notifications
such
as IN's, SOER's,
and SIL's; and,
the completion of actions
in followup to your
1987 maintenance
program self assessment.
Please
provide
a written response
to
this office within 45 days of your plans to resolve
these
items.
The enclosed
report presents
the details of the inspection
and includes
an executive
summary
highlighting the significant inspection findings.
